院前死亡患儿的原因及影响复苏的因素分析

摘　　要：目的分析院前死亡（DOA）患儿的原因，探讨影响复苏的因素。方法收集2009年1月至2012年3月本院急诊科收治的30例院前死亡患儿的临床资料，对其进行回顾并分析探讨。结果30例患儿入院时均已停止呼吸心跳，13例患儿停止时间明确，17例患儿停止时间不详。患儿家属送入11例，外院转入19例。30例患儿入院后均进行复苏抢救，建立静脉通道，注射肾上腺素等进行抢救，但由于呼吸心跳停止时间过长，30例患儿均抢救无效死亡。院前死亡患儿的病因以次为肺炎合并呼吸衰竭或心力衰竭15例，先天性心脏病例6例、窒息4例、颅内感染3、意外伤与中毒2例。结论普及全民心肺复苏技术，加强儿童卫生保健，完善儿童急诊医疗体系，根据不同患儿死因特点，采取不同的救治措施是减少院前死亡率和提高抢救成功的关键。Objective analysis of death before hospital （DOA） in children with reason, to explore the factors affecting recovery. Methods the clinical data of 30 cases of death were collected from 2009 January^2012 year in March the hospital emergency department were reviewed and analyzed before, and to explore the. Results 30 cases of children in the hospital have stopped breathing and heartbeat, 13 cases of children with stopping time clearly, 17 cases of children with stopping time is unknown. Family members into 11 cases, 19 cases were transferred from other hospitals. 30 cases of children admitted to hospital were resuscitation, establish vein channel, injection of epinephrine to rescue, but because of respiratory and cardiac arrest time is too long, 30 cases were died. Pre-hospital death causes of children with pneumonia and respiratory failure or heart failure in 15 cases, cases of congenital heart disease in 6 cases, 2 cases of 4 cases of asphyxia, intracranial infection in 3, injury and poisoning accident. Conclusion：Universal CPR techniques, strengthen children health care, improve children＇s emergency medical service system, according to the different characteristics of children death, take treatment measures of different is to reduce the pre-hospital mortality and improve the successful key.
