血浆D-二聚体、纤维蛋白原和β_2-微球蛋白三项联检在恶性肿瘤中的应用

摘　　要：目的探讨血浆D-二聚体、纤维蛋白原和β2-微球蛋白含量与恶性肿瘤的相关性及观察恶性肿瘤组中治疗前后三者水平变化与疗效及预后的关系。方法取110例常见恶性肿瘤患者以及50名健康对照组的静脉血,血浆D-二聚体和纤维蛋白原含量用日本Sysmex CA-1500全自动血凝仪测定,血浆β2-微球蛋白含量用日本Hitachi7600-020全自动生化分析仪检测。结果 110例恶性肿瘤患者中肿瘤初治组和难治转移组较正常对照组的血浆D-二聚体、纤维蛋白原和β2-微球蛋白含量明显升高(P<0.05),治疗缓解组的血浆D-二聚体、纤维蛋白原和β2-微球蛋白含量均有不同程度的下降(P<0.05)。结论血浆D-二聚体、纤维蛋白原和β2-微球蛋白含量变化与恶性肿瘤及其预后显著相关,三者联合检测可提高恶性肿瘤的诊断可靠性,减少漏诊率和误诊率,并可作为恶性肿瘤治疗前后疗效对比及预后判断的一项早期参考指标。Objective To discuss the correlation of plasma D-dimer,fibrinogen and β2 microglobulin with cancer and observe the pre-and post-treatment level changes of these three indexes and the relation between curative effect and prognosis.Methods The venous blood from 110 patients with common cancers and 50 healthy persons in the control group was taken.The contents of plasma D-dimer and fibrinogen were determined by Japan Sysmex CA-1500 automatic coagulometer and β2 microglobulin by Japan Hitachi7600-020 automatic biochemical analyzer.Results In comparison with the normal controls,the contents of plasma D-dimer,fibrinogen and β2 microglobulin in the initial treatment cancer group and the difficult-to-treat and metastasis cancer group increased obviously（P〈0.05）,but decreased in different degrees in patients whose cancer was in remission（P〈0.05）.Conclusion The content changes of the plasma D-dimer,fibrinogen and β2 microglobulin are obviously related with the cancer and its prognosis.The joint detection of the three items can be used to improve the diagnosis reliability so that the rate of missed diagnosis and misdiagnosis can be reduced greatly.Meanwhile,the joint detection can also be taken as an early reference index for the pre-and post-treatment effects and the contrast prognosis judgment for the cancer treatment.
