经皮CT引导下^(125)Ⅰ粒子植入联合PC方案治疗局部晚期非小细胞肺癌34例疗效观察

摘　　要：目的评价CT引导125I粒子植入联合PC方案治疗局部晚期非小细胞肺癌的可行性及疗效。方法 34例初治肺癌患者均行经皮CT引导下125I粒子植入后5～7 d给予PC方案:PTX175 mg/m2,IV.(3h inf),第1天;CBP(采用AUC给药)5 mg.ml-1.min-1,IV,第2天。28 d为1个周期,化疗2～4周期。125I粒子植入8周后进行疗效及不良反应评价。结果125I粒子植入后3例死亡,32例进入疗效和不良反应评价。全组CR 3例,PR 19例,SD 6例,PD 4例。有效率(CR+PR)/(CR+PD+SD+PD)=71%。结论125I粒子植入联合PC方案化疗对局部晚期NBCLC患者是一种有效、安全的治疗方法。Objective To evaluate the feasibility,safety and curative effect of radioactive particle 125I combined with PC in treating locally advanced non-small cell lung cancer.Methods 34 cases of primarily treated patients were embedded with 125I particles into tumors under the guidance of CT and then treated with PC regiments after 5-7 days.The PC regiments included：PTX 175mg/ m^2,intravenous dripping in 3 hours,on the 1st day;CBP（AUC） 5mg/ml/min,intravenous dripping,on the 2nd day;28 days as one cycle,altogether 2-4cycles.8 weeks after the embedment,the short term and adverse effects were evaluated.Results Two months afterwards,3 patients died and 31cases showed curative effects（CR in 3 cases,PR in 19 cases,SD in 6 cases,and PD in 4 cases）.The effective rate was 71%（（CR＋PR）/（CR＋PD＋SD＋PD）=71%）.Conclusion The therapy of embedding radioactive particle 125I combined with PC is effective and safe for the treatment of locally advanced non-small cell lung cancer.
