摘 要:目的研究用免疫组化方法检测胃癌淋巴结中微转移的临床病理意义.方法作者共检测了42例胃癌的1251只淋巴结,所有的肿瘤组织和淋巴结均分别用 HE 染色和抗角蛋白19抗体及抗 CEA抗体的免疫组化染色.结果用 HE 染色发现有377只(30.1%)淋巴结转移阳性,免疫组化染色发现共有463只(37.0%)淋巴结转移阳性,其中86只淋巴结只有微转移.42例胃癌中有19例患者的淋巴结中发现微转移,其中3例仅有微转移,占常规病理检查淋巴结转移阴性患者的27.3%(3/11).在弥漫型胃癌以及侵及浆膜的胃癌中,微转移阳性率显著高于其他肿瘤.结论免疫组化染色是检测胃癌淋巴结微转移的敏感方法,检测胃癌微转移有助于判断肿瘤进展程度.AIM The clinicopathologic significance of micrometastasis to lymph nodes in patients with gastric carcinomas was studied by immunohistochemical methods. METHODS The authors studied 1251 lymph nodes in 42 patients with gastric carcinomas.Histologic sections of the primary tumor and regional lymph nodes in each case were stained with hematoxylin and eosin (HE) and immunostained with antibody against cytokeratin 19 (CK19) or carcinoembryonic antigen (CEA),respectively. RESULTS Metastases were confirmed in 377 lymph nodes (30.1%) stained with HE,and in 463 lymph nodes (37.0%) stained with immunohistochemistry. Micrometastases were found in 86 lymph nodes.In 42 gastric cancer patients,19) cases had micrometastases in lymph nodes,but 3 patients had only micrometastases, which occupied 27.3% (3/11) of patients with negative findings detected by routine method.Micrometastases in lymph nodes were significantly related to the depth of invasion and Lauren type. CONCLUSION CK19 and CEA:Immunohistochemical staining is a sensitive method for the detection of lymph nodes micrometastases in patients with gastric carcinomas.Lymph nodes micrometastases are significantly correlated with tumor progression.
