摘  要:目的研究氧自由基清除药(依达拉奉)对急性脑出血患者血清炎症因子的影响。方法将120例急性脑出血患者按照随机对照原则分为治疗组和对照组,每组60例。2组患者均接受脱水,降颅内压,控制血压、血糖,以及改善脑细胞代谢和对症支持治疗,治疗组患者在此治疗基础上给予依达拉奉治疗,观察2组患者的血清炎症因子水平,并评价临床疗效及预后。结果 2组患者治疗后血清白介素6(IL 6)、肿瘤坏死因子α(TNFα)、内皮素(ET)、细胞间黏附分子1(ICAM 1)和单核细胞趋化因子1(MCP 1)水平均较治疗前明显降低;治疗后,治疗组患者IL 6、TNFα、ET、ICAM 1和MCP 1水平均显著低于对照组。2组患者治疗后神经功能缺损量表(NIHSS)及改良Rankin量表(mRS)评分均较治疗前明显降低,而日常生活活动能力(ADL)评分明显升高;治疗后,治疗组NIHSS及mRS评分显著低于对照组,而ADL评分显著高于对照组,差异均有统计学意义。结论氧自由基清除药可有效抑制急性脑出血患者的炎症反应,提高临床疗效,改善患者预后。Objective To explore the effects of an oxygen free radical scavenger on serum inflammatory factors of patients with acute cerebral hemorrhage.Methods One hundred and twenty patients with acute cerebral hemorrhage were randomly divided into treatment group and control group,60 cases in each group.The patients in both groups were given for treatment dehydration,decrease of intracranial pressure,control of blood pressure and glucose,improvement of the metabolism of brain cells as well as support therapy.Based on this treatment,the patients in the treatment group were treated with edaravone.The levels of serum inflammatory factors in patients of the two groups were observed,and their clinical effects and prognosis were evaluated.Results After treatment,the levels of serum interleukin 6 (IL-6),tumor necrosis factor (TNF-α),endothelin (ET),intercellular adhesion molecule-1 (ICAM-1) and monocyte chemotactic protein-1 (MCP-1) in patients of both groups obviously decreased compared with before treatment,and those in the treatment group were significantly lower than in the control group.After treatment,the scores of NIHSS and modified Rankin scale (mRS) significantly reduced compared with before treatment,while the activity of daily living (ADL) score markedly increased; NIHSS and mRS scores in the treatment group were significantly lower than in the control group,while ADL score remarkably higher than the control group,and both differences had statistical significance.Conclusion Oxygen free radical scavengers can effectively inhibit the inflammatory reactions of patients with acute cerebral hemorrhage,enhance the clinical effects and prognosis.
