放疗、化疗综合治疗对局部中晚期鼻咽癌患者营养状态的影响

摘　　要：目的评估放化疗综合治疗对局部中晚期鼻咽癌患者营养状态的影响。方法观察50例接受放化疗综合治疗的局部中晚期鼻咽癌患者放疗前后体质量、体质量指数(BMI)、血常规、急性放射黏膜反应情况,应用营养风险筛查(NRS)2002评估表评价患者的营养状态。结果治疗后,患者体质量、BMI、白细胞计数(WBC)、红细胞计数(RBC)、血红蛋白(Hb)均较治疗前明显降低,而NRS评分较治疗前明显升高。接受普通放疗的患者出现Ⅱ度以上黏膜反应9例(90%),接受调强放疗的患者出现Ⅱ度以上黏膜反应22例(55%),二者比较差异显著。所有患者有轻度胃肠道反应,18例(36%)出现Ⅱ度胃肠道反应,无1例发生Ⅲ度以上胃肠道反应。结论鼻咽癌患者放化疗时体质量下降及骨髓抑制对营养风险的发生影响较大,NRS 2002能较客观地反映被测者的营养风险,以便及早进行营养干预。Objective To evaluate the influence of chemoradiotherapy on the nutritional status of patients with local middle-advanced nasopharyngeal carcinoma （NPC）.Methods The body weight,body mass index （BMI）,blood routine and acute radioactive mucous reaction of 50 patients with local middle-advanced NPC treated with chemoradiotherapy were observed,and the nutritional status of patients was evaluated by nutritional risk screening （NRS） 2002.Results After treatment,the body weight,BMI,white blood count （WBC）,red blood count （RBC） and hemoglobin （Hb） of patients obviously decreased compared with before treatment,while NRS score significantly increased.Those treated by common radiotherapy suffered from mucous reactions over grade Ⅱ were 9 （90％）,while （55 ％） those treated with intensity modulated radiation therapy （IMRT） from mucous reactions over grade Ⅱ were 22 patients.Significant difference was presented.All patients had mild gastrointestinal reactions,18 cases suffered from grade Ⅱ gastrointestinal reactions,and no one suffered from grade Ⅲ gastrointestinal reactions or above.Conclusion Decreased body weight and bone marrow suppression greatly affect the occurrence of nutritional risk when patients with NPC receive chemoradiotherapy.NRS 2002 can objectively reflect the nutritional risk for the subjects so as to perform the nutritional intervention as soon as possible.
