自体巩膜瓣在眼球摘除联合羟基磷灰石义眼座植入术中的应用

摘　　要：目的探讨自体巩膜瓣覆盖羟基磷灰石(HA)义眼座前部对预防术后眼座暴露的临床效果。方法眼球摘除后,取自体巩膜瓣覆盖缝合固定于I期植入的HA义眼座前部。结果所有病例随访6～18个月,均无义眼座暴露发生。结论在行HA义眼座植入术时,使用自体单层巩膜瓣覆盖义眼座前部,能阻止义眼座的粗糙表面对前部眼球筋膜囊和球结膜持续的机械刺激,避免眼球筋膜囊和球结膜裂开,有效地预防义眼座暴露。Objective To study the application of autologous sclera flap covering to prevent exposure of hydroxyapatite prosthesis seat after eyeball removal.Methods Implanted hydroxyapatite prosthesis was covered with autologous scleral flap sutured in front of the seat in phase I.Results In all 21 cases that were followed up for 6 to 18 months,no orbital implantation exposure has occurred.Conclusion IAutologous monolayer sclera flap can be used to cover the front of implanted hydroxyapatite prosthesis.This method could prevent the irritation or tearing of the anterior facials and bulbar conjunctiva caused by constant contact to a rough surface of the prosthesis seat,and therefore could effectively prevent the exposure of the implanation.
