卡前列素氨丁三醇在腹腔镜子宫肌瘤剔除术中的应用

摘　　要：目的观察在腹腔镜子宫肌瘤剔除术(laparoscopic hysteromyomectomy,LM)中使用卡前列素氨丁三醇的效果,评估其在LM中的应用价值。方法将60例子宫肌瘤患者分为两组,观察组30例在术前30min肌肉注射卡前列素氨丁三醇250μg,对照组30例在术中同时给予缩宫素(Oxytocin,OT)20U静脉维持滴注,比较两组患者术中出血量、手术时间、术中不良反应发生率、术后24h基本情况的差异。结果观察组术中平均出血量明显减少,手术时间明显缩短(P<0.05)。两组患者术中不良反应发生率、术后24h基本情况比较差异无统计学意义(P>0.05)。结论与术中应用OT相比,LM前应用卡前列素氨丁三醇,在减少术中出血、缩短手术时间方面具有明显优势。Objective To evaluate the value of Romethamine（Hemabate） used in the operation of laparoscopic hysteromyomectomy（LM） , by observing Hemabate used in the operation of LM. Methods Total 60 specimens patients with LM were divided into two groups with 30 patients in each group. Intramuscular injection of Hemabate 30 -rain before LM was used in 30 specimens patients of observation group, while intravenous administration of oxytocin during operation was used in the other 30 specimens patients of control group. Differences of the volume of bleeding in operation, the total time of operation, the incidence of adverse reactions in operation, and 24 h general conditions of patients after surgery were collected. Results Compared with control group, patients in observation group had less volume of bleeding and shorter total surgery time. No difference was observed between two groups about the incidence of adverse reactions in operation and 24 h general conditions of patients after surgery. Conclusion Compared with using oxytocin in LM, hemabate used before the surgery has obvious advantage in reducing blood loss and shortening surgery time.
