摘　　要：2009年起,上海在三级综合医院中试点医保总额预付,并于次年扩大试点范围,2011年在二级以上医院全面实施。医院一方面坚持"发展是硬道理"的理念,一方面采取相应的应对措施以适应新医保支付制度带来的挑战。本文总结了医院的应对策略、医院运行出现的新变化和相关政策建议,以期对医院管理者和医保部门以启示。Global budgeting was piloted in general tertiary hospitals of Shanghai in 2009.Two years later,all the secondary and tertiary hospitals in Shanghai were included in this new health insurance payment reform.The hospitals tried it best to take corresponding measures to meet the health demands of the users,maintain the normal operation,as well as meet the challenge of the new policy.This paper summarizes the coping strategies,operational problem of the new payment policy and provides recommendations to hospital managers and health insurance government for reference.
