胃上皮性肿瘤细胞三维结构的体视学参数的判别分析

摘　　要：目的探讨胃的良恶性上皮性肿瘤的三维结构的体视学参数,筛选对其诊断有重要意义的参数。方法收集82例,病例包括:胃腺癌55例,胃腺瘤8例,胃腺癌患者癌旁正常胃粘膜19例。石蜡切片,HE染色。每例随机取10个视野,用计算机图像分析技术测试有关细胞核和腺体的体视学参数。结果有关体视学参数所建立的判别函数对胃腺癌和胃腺瘤及正常胃组织的判别准确度分别为97.1%、87.5%和100%;对胃高分化腺癌、中分化腺癌、低分化胃腺癌及正常胃组织的判别准确度分别为53.3%、70%、95%和84.2%。结论三维结构的体视学参数Vvn.epi、Svn.epi、Rsvn、λepi、Vn,epi、Sn,epi、Nvn,epi、Rnp及NLS对胃上皮性肿瘤及其胃腺癌分级的判别和诊断具有重要的价值。Objective To explore the three-dimensional structure of the stomach epithelium neoplasm. And to screen highly specific parameter for diagnosis purpose. Methods A total of 82 stomach cases were chosen including 55 stomach car- cinoma,8 stomach adenoma and 19 normal stomach specimens. Ten fields were selected stochastically from each sections cut from three paraffin masses selected randomly and stained by HE. Results Using the discriminate function to differentiate the stomach carcinoma and the normal stomach group with the differentiating accuracy of 97.1%, 87.5% and 100%, respectively. The differentiation rates of the welldifferentiated adenocarcinoma, moderately differentiated adenocarcinoma, low differentiated adenocarcinoma and normal tissues were 53.3% ,70%,95%和 84.2%, respectively. Conclusions Three dimensional struc- ture parameters of Vvn.epi、Svn.epi、Rsvn、λepi、Vn,epi、Sn,epi、Nvn,epi、Rnp possessed important value in differentiating diagnosis of gastric epithelial neoplasm and gastric gland cancer classification.
