硬腭粘膜联合唇粘膜移植治疗完全性睑球粘连

摘　　要：目的 探讨硬腭粘膜联合唇粘膜移植治疗完全性睑球粘连的效果及临床价值。方法选择１２例１３只眼外伤后完全性睑球粘连病例，先行睑球粘连分离，切取硬腭粘膜及后粘膜，以硬腭粘膜再造眼睑后层，唇粘膜再造穹窿及球结膜，置入适宜的眼模后行睑缘缝合。术后半年睑缘切开后酌情行板层角膜移植、配戴美容接触镜或超薄义眼壳。结果 全部病例随访超过６个月，手术效果优８只眼、良３只眼、差２只眼。移植硬腭粘膜病理检查显示：其为６～７复层鳞状上皮，上皮下胶原纤维排列致密整齐与睑板结构相似。结论硬腭粘膜联合移植治疗完全性睑球粘连，可一次修复睑结膜及睑板，减小穹窿收缩，明显地改善了睑球粘连手术的治疗效果。Objective The study focus on evaluation the utilization of combined transplantation of hard palate and lip mucous for treating completed symblepharon. Methods The objects were 12 patients with 13 sick eyes. After symblepharon being separated,hard--palate and lip mucous were harvested. The tarsal plate and palpebral conjunctive were reconstucted using hard--palate mucous,while,the reformation of fornix and bulbous conjunctiva using lip mucous. Adhesion the eyelid edges was done routinely after placing a fit plastic eye shell. Six months late,eyelid adhesion was released,then the corneal lamelar graft, cosmetic contact lens or ultrathin prosthesis were selected individually. Results Following up over 6 months,8 eyes were excellent,3 eyes were good and 2 failed. Histologic observation of grafted hard-palate mucous revealed 6 to 7 layers of squamosal epithelium which also had similar tighten subepithelial collagen fibrils of tarsal plate. Conclusion The combines ransplantation of hard--palate and lip mucous can supply a reconstruction of palpebral conjuntiva and tarsal plate on patients with completed symbleplaron in only one step. It could greatly decrease fornix contraction and improve the surgical outcome of symblepharon obviously.
