摘　　要：目的探讨弹性纤维瘤(elastofibroma)的诊断及治疗方法。方法回顾性分析1999年1月至2012年12月间收治的18例弹性纤维瘤的临床资料,总结弹性纤维瘤的临床特点、影像学表现、病理改变和近/远期手术疗效。结果本组18例,男3例,女15例,平均年龄65.2岁。发病部位均为肩胛下角。右侧病变8例(44.4%),左侧病变6例(33.3%),双侧病变4例(22.2%)。典型的症状表现为后背肩胛下角处肿物,可伴有局部疼痛不适及活动肩关节时弹响感等。根据临床特点和特征性的影像学可明确诊断。手术方式为肿物切除术。术后随访4～104个月(平均46.7个月),无复发。结论弹性纤维瘤常见于老年女性,多发于肩胛下角区域。正确诊断该病依靠典型体征及影像学特征性表现。手术切除后近/远期效果良好。Objective To explore the experience in the diagnosis and treatment of elastofibroma dorsi. Method Clinical data of 18 cases of pathologically confirmed elastofibroma from January 1999 to December 2012 were reviewed. The clinical features, the radiological characteristics, the pathological alterations and the short-term and long-term effects were analyzed. Result 18 patients （3 males and 15 females; mean age： 65.2 years） with elastofibroma diagnosis were surgi- cally treated at our institution. Elastofibroma was right sided in 8 patients （44. 4% ） , left in 6 （33.3%） , and bilateral in 4 （22. 2% ）. Clinical presentation included a mass in the subscapular region, a clunking sensation or a localized scapu- lar swelling during the shoulder movements. Accurate diagnosis was made solely based on the physical examination and im- aging findings. All patients underwent surgical marginal excision. No sign of recurrence was found during the follow-up pe- riod （4 months to 104 months）. Conclusion Elasto^ibroma typically occurs in the snbscapular region of elderly women. The diagnosis can be made on the basis of unique imaging characteristics and physical examination before histological exam- ination. Surgical marginal excision is the best treatment and can achieve good short-term and long-term effects.
