胰头部胆总管周围微小内分泌肿瘤临床病理研究

摘　　要：目的对胰头部胆总管周围微小内分泌肿瘤的临床病理学特点、诊断、鉴别诊断及患者的预后等进行探讨。方法对标本进行常规石蜡切片及HE染色,光镜观察,并对石蜡切片做免疫组化染色。结果患者为男性,56岁,临床表现主要为皮肤和巩膜黄染及皮肤瘙痒10余天。巨检见胰头部质地坚硬,呈灰白色,胆总管下段明显狭窄。显微镜下见在胰头内及胆总管周围大片增生的纤维组织中有多量成巢及成簇分布的内分泌性小细胞增生,并可见内分泌细胞围绕着小神经呈浸润性生长。免疫组化染色见瘤细胞表达CgA、Syn、胰岛素及生长抑素等阳性。结论本例病变的性质可能属于最早期的潜在恶性的内分泌肿瘤,本文尚对肿瘤的诊断依据及鉴别诊断等进行了探讨。Objective To probe the clinical pathological features,diagnosis and differential diagnosis and the prognosis etc.of the tiny endocrine tumor in the head of pancreas and the peripheral position of common bile duct.Methods The sample was section by using paraffin slice and HEstaining,observed under microscope.And the paraffin slices were stained with Immunohistochemistry method too.Results The malepatient was 56 years old.The main clinical manifestations were yellowing of the skin and sclera,and felt itch.Macroscopically,the head of pancreas proved stiff and gray,and obviously narrowedat the end of common bile duct.Under the microscope,we can found endocrine small cells which characteristic as nests and clusters located in proliferated fibrous tissue at the head of pancreas and the peripheral position of common bile ductproliferated.And endocrine cells infiltrated proliferated at the peripheral part of the small nerves.CgA,Syn,insulin and somatostatin were overexpressed in tumor cells.Conclusions The lesion of this case might belong to the earliest endocrine tumor which in potentialmalignant.In this article,we also discussed the diagnosis and differential diagnoses for tiny endocrine tumor.
