无痛结肠镜下大肠息肉切除1229例临床效果观察

摘　　要：结肠镜下进行大肠息肉切除共2 256例,分为无痛组1 229例和常规组1 027例。无痛组治疗时间为(14.2±2.8)min,常规组为(18.1±3.2)min(P<0.05);成功率无痛组为97.25%,常规组为92.36%,(P<0.05);满意程度无痛组明显提高(P<0.05);腹痛腹胀、血管迷走反射和术中体动等不良反应无痛组显著低于常规组(P<0.01);舌后坠、头晕等不良反应无痛组显著高于常规组(P<0.01或0.05);肠道穿孔等严重并发症发生率两组无明显差异。Objective To evaluate the clinical success rate and security of removing colorectal polyps by painless colonoscope. Methods 2256 cases of patients with colorectal polyps were divided into Analgesia group （ 1229 cases） and Conventional group （1027 cases）according to whether accepting painless technique or not during operation. The treatment time, success rate, adverse reaction, satisfaction rate were recorded. Results The average treatment time and success rate were （14.2±2.8 ） min and 97.25% in Analgesia group and （18.1±3.2） min and 92.36% in Conventional group, respectively. The differences of treatment time and success rate between the two groups were statistical significant （ P 〈 0.05 ）. The satisfaction rate in analaesia group significantly higher as compared to that in conventional group （ P 〈 0.05 ） ; adverse reactions in analgesia group such as al^dominal pain, abdominal distension, vasovagal reaction and body movement during operation were significant less than those in conventional group（ P 〈0.05）. However, glossocoma and dizziness in analaesia group were significantly more than those in conventional group（ P 〈0.01 or 0.05 ）. There was no obvious difference in serious complications rate be- tween the two groups. Conclusion Removing colorectal polyps through painless colonoscope can shorten curative time, increase the success rate, reduce adverse reaction and improve satisfaction rate of patients. In addition, it should improve intensive care during operations.
