卵巢恶性肿瘤临床特征及预后因素分析

摘　　要：目的:总结卵巢恶性肿瘤患者的临床特点及预后情况并探讨卵巢恶性肿瘤预后相关因素。方法:回顾性分析123例卵巢恶性肿瘤患者临床资料并进行随访,采用Kaplan-Meier法及cox多因素生存分析来分析预后影响因素。结果:123例卵巢恶性肿瘤患者中早期(I期+II期)73例,晚期(III期+IV期)50例;浆液性肿瘤60例,非浆液性肿瘤41例,非上皮性肿瘤22例;所有患者1、3、5年生存率分别为94%、77%、71%。多因素分析显示年龄、临床分期、组织学分级、淋巴结清扫情况是卵巢恶性肿瘤独立预后因素。结论:年龄、临床分期、组织学分级、淋巴结清扫情况是卵巢恶性肿瘤独立的预后因素,早期发现、早期诊断、合理治疗是提高患者生存率的关键。Objective： To summarize the clinical characteristics and prognosis of patients with ovarian malignant tumors, explore the related influencing factors of prognosis of ovarian malignant tumors. Methods： The clinical data of 123 patients with ovarian malignant women were analyzed retrospectively and followed up; Kaplan - Meier survival analysis and Cox multiple factor survival analysis were used to analyze the influencing factors of prognosis. Results： Among 123 patients with ovarian malignant tumors, 73 patients were in the early stages （stage Ⅰ, Ⅱ ） and 50 patients were in the advanced stages （stage Ⅲ, Ⅳ） ; 60 patients were found with serous ovarian tumors, 41 patients were found with non - serous ovarian tumors, and 22 patients were found with non - epithelial ovarian tumors ; the one - year, three - year, and five -year survival rates of all the patients were 94% , 77% , and 71% , respectively. The resuhs of multivariate logistic regression anal- ysis showed that age, clinical stages, histological grades, and the status of lymph node dissection were independent prognostic factors of ovar- ian malignant tumors. Conclusion： Age, clinical stages, histological grades, and the status of lymph node dissection were independent prognostic factors of ovarian malignant tumors ; early detection, early diagnosis, and reasonable treatment are key points to improve survival rate of patients with ovarian malignant tumors.
