超声乳化及人工晶体植入术治疗原发性闭角型青光眼并白内障的临床疗效观察

摘　　要：【目的】探讨单纯白内障超声乳化及人工晶体植入术治疗原发性闭角型青光眼合并白内障患者的疗效。【方法】对50例60眼原发性闭角型青光眼并白内障患者采用白内障超声乳化及人工晶体植入术治疗，观察患者术前，术后眼压及视力变化，并用超声生物显微镜（UBM）检查眼前节的情况。【结果】①术后1周、1个月患者的眼压均较术前明显降低，且差异均有显著性（P〈0．05），术后1周与1个月相比眼压差异无显著性（P〈0．05）。术后1个月时最佳矫正视力平均为1．0±0．36，较术前（O．16±0．16）明显提高，且差异均有显著性（P〈0．05）。术后1周时发现1例眼压高于21mmHg，为囊袋内植入，术后1个月时有2例眼压高于21mmHg，均为人工晶体不对称患者。②术后l周发现少量皮质残留1眼（2％），术后1个月时无明显变化。【结论】①白内障超声乳化及后房型人工晶体植入术能显著降低患者的眼压，改善视力。②局限性房角狭窄及少量皮质残留不会导致术后患者的长期高眼压。[Objective] To explore the efficacy of simple cataract phacoemulsification and intraocular lens im- plantation（IOL） for the treatment of primary angle-closure glaucoma with cataract. [Methods]Totally 50 patients （60 eyes） with primary angle-closure glaucoma and cataract were treated with cataract phacoemulsification and IOL. The change of intraocular pressure and eyesight before and after operation were observed. Ultrasonic biomi- croscope（UBM） was used to examine the anterior segments of eyes. [Results] Compared with before operation, intraocular pressure of patients at 1 week and 1 month after operation obviously decreased, and there was signifi- cant difference（ P 〈0.05）, but there was no significant difference in intraocular pressure between at 1 week and 1 month after operation（ P 〉0.05）. Compared with before operation, the best corrected vision at 1 month after op- eration obviously increased（1.0±0.36 vs 0. 16±0.16）, and there was significant difference（ P 〈0.05）. Intraocu- lar pressure of one patient at 1 week after operation was higher than 21mmHg, and the patient was sac implantation. Intraocular pressure of 2 patients at 1 month after operation was higher than 21mmHg, and the patients were asymmetry intraoeular lens. Small amount of residual cortex was found in one eye（2 %） at 1 week after operation, but there was no change at 1 month after operation. [Conclusion]Cataract phacoemutsification and posterior cham- ber IOL implantation can markedly decrease intraocular pressure and improve the eyesight. Local narrow angle and small amount of residual cortex don＇t cause the long-term ocular hypertension of patients after operation.
