两种微螺钉支抗正畸方法为种植义齿提供修复空间的临床研究

摘　　要：目的探讨为磨牙过度萌长患者提供种植修复空间的两种正畸方法的优缺点。方法选取63例由于长期牙缺失导致对颌牙过度萌长并要求种植修复的患者,根据患者意向及是否伴有其他错牙合畸形分为两组:39例单纯采用微螺钉支抗压低过度萌长牙(单纯组),另外24例采用微螺钉支抗压低过度萌长牙的同时伸长其余磨牙(联合组)。对两组的成功率、疗程、力值和牙根吸收情况分别进行研究。结果在单纯组和联合组,牙齿平均压低量分别为3.9 mm和2.7 mm;矫治有效率分别为89.7%和100%;目标牙平均根吸收值分别为1.1和0.7;平均疗程分别为8.2个月和17.4个月;每牙平均矫治力值分别为87 g和79 g。结论单纯组的优点为美观性好、疗程短、费用相对低、无需保持,但存在失败率高、牙根吸收较多的缺点;联合组虽有成功率高、牙根吸收较少、可同时解决多种错牙合畸形的优点,但其美观性欠佳、疗程长、费用相对高、需戴用保持器的缺点却不易被大多数人所接受,故应根据患者意愿及临床具体情况制定矫治方案。Objective To investigate the advantages and disadvantages of two orthodontic methods using micro-screw anchorage for producing dental implant space. Methods Sixty-three patients with overextruded molar teeth caused by long-term teeth defect were selected and divided into two groups according to patient＇ s intention and malocclution. Teeth of 39 patients were corrected by orthodon- tic micro-implant anchorage to intrude the overgrowing molars（ single treatment group）. Teeth of 24 patients were corrected by intruding the overgrowing molar and extruding the other molars （combination treatment group）. The orthodontic effective rate, orthodontic root re- sorption rate, average treatment period were compared between two groups. Results The orthodontic effective rate was 89.7% and 100% in single treatment group and combination treatment group, respectively, the root absorption value was 1.1 and 0.7 respectively, the average treatment period was 8.2 months and 17.4 months respectively, and the average orthodontic force of every tooth was 87 g and 79 g respectively. Conclusion Both two orthodontic methods have advantages and disadvantages. The single treatment has the ad- vantages of beautiful appearance, shorter treatment period and lower medical fee, but it has higher failure rate and more serious tooth root absorption. The combination treatment has advantages of lower treatment failure rate, less serious teeth root absorption and being a- ble to correct multi-malocclution simultaneously, but it is of ugly appearance, longer treatment period, higher medical fee and wearing re- tainer. The proper orthodontic method should be chosen individually according to patient＇ s intention and malocclusion.
