精神活性物质所致精神病性障碍患者预后的研究

摘　　要：目的:探讨精神活性物质所致精神病性障碍患者的预后。方法:2012年1～3月应用一般资料调查表及阳性与阴性症状量表(PANSS)对2003年1月至2010年12月因精神活性物质所致精神病性障碍住院的189例患者进行随访。结果:随访时复吸率为88.9%;随访前6个月内仍吸毒32例(16.9%)。PANSS评估示42例(22.2%)随访时有不同程度精神病性症状,其中19例仍吸毒。根据PANSS评估将随访前6个月无吸毒的157例患者分为有精神病性症状组(23例)及无精神病性症状组(134例);与无精神病性症状组比较,有精神病性症状组初吸年龄小、吸毒年限长、家族精神病阳性率及再住院率高(P均<0.05)。结论:精神活性物质所致精神病性障碍患者多数预后较好,预后不良者可能与其初吸年龄、吸毒年限及家族精神病史有关。Objective：To investigate the prognosis of patients with psychotic disorders due to psychoactive substance abuse.Method：January to March in 2012 the follow-up was preformed in 189 patients with psychotic disorders due to psychoactive substance abuse and were hospitalized during January 2003 to December 2010 by use general information questionnaire and positive and negative symptoms scale （PANSS） .Results：The relapse rate was 88.9% when the follow-up,and there was still a drug use in 32 cases （16.9%） during the 6 months before the follow-up.42 cases （22.2%） still had psychotic symptoms in different degree by PANSS assessment,including 19 cases had still taking drugs.The 157 cases who without drug abuse in 6 months before the follow-up,were divided into the psychotic symptom group （23 cases） and non psychotic symptom group （134 cases） according to PANSS assessment.Compared with non psychotic symptom group,the age of the first time drug use was small,the number of year for drug use was longer,and the rate of family psychosis positive and rehospitalization in psychotic symptom group were higher significantly（all P0.05）.Conclusion：The prognosis in patients with psychotic disorders due to psychoactive substances abuse is good.The patients with poor prognosis may have early age,longer drug use and family history of psychotic illness.
