摘 要:目的探讨中枢神经系统原始神经外胚层肿瘤(CNSPNET)的CT、MRI特征,以此提高对CNSPNET的CT、MRI诊断准确性。方法回顾性分析9例CNSPNET的CT及MR资料.并复习相关文献。结果本组9例CNSPNET,男7例,女2例;20岁以下5例;病灶位于大脑半球6例,脑f、松果体区及基底节各1例;8例为囊实性,1例为实性;病灶实性部分在CT上以稍高密度呈现,T1WI以等或稍低信号呈现。T1WI上以不均匀等或稍高混杂信号呈现;增强扫描均为强化明显,呈环状、蜂窝状;5例伴瘤周轻度水肿,6例边界清晰,4例出血,2例钙化,4例出现血管流空影;4例贴近脑膜,其中1例出现脑膜尾征。结论CNSPNET各年龄段均可发病,肿瘤实性部分为稍高密度,MR信号混杂,病灶血供丰富,占位效应明显,但瘤周水肿一般较轻,边界尚清。联合应用多种影像学技术可提高其术前诊断准确性。Objective To investigate CT, MRI features of primitive neuroectodermal tumors of central nervous system(CNS PNET) in order to improve the diagnostic accuracy of CT, MRI in CNS PNET. Methods CT and MR imaging data of 9 cases of CNS PNET were analyzed retrospectively,and the related literature were reviewed. Results 9 cases of CNS PNET were collected, including 7 male patients and 2 female patients. The age of onset of 5 cases were below 20 years old. The location of the tumor was listed as following: 6 cases in the cerebral hemisphere, 1 case in the brainstem, the pineal region and the basal ganglion respectively. Cystic changes were found in 8 cases. CT showed that solid component of all lesions was slightly hyperdense, whose signal were iso-intense or hypointense on MRI TI WI and heterogeneously iso-intense or slightly hyperintense on MRI T2 WI. All of lesions showed obvious enhancement and looked like a loop or honeycomb after contrast-enhancement. The borders of these tumors were well-defined in 6 cases. Slight peritumoral edema was seen in 5 cases, hemorrhage in 4 cases, calcifications in 2 cases, and vascular flow void in 4 cases. The lesions were very close to dura mater in 4 cases and dural tail sign appeared in 1 case. Conclusion CNS PNET can occur in every age. The solid component of lesion is slight hyperdensity, and their MRI signal intensities are inhomogeneous. The blood supply of lesion is rich, and the mass effect is obvious, but the peritumoral edema is mild and the mass is well-defined. The diagnostic accuracy of CT, MRI in CNS PNET can be improved with combination of multiple medical imaging technology.
