摘 要:目的探讨64层螺旋CT对股骨近端肿瘤及肿瘤样病变的诊断价值。方法对20例股骨近端肿瘤及肿瘤样病变同时行X线平片检查及64层螺旋CT扫描,CT扫描后对原始数据进行薄层重组及MPR重建,比较x线平片、CT横断位及MPR3种图像评价方法对病变特征的显示能力及诊断符合率。结果3种影像学评价方法中,MPR对病变范围的估计及病变的各种特征(包括硬化边、瘤骨/钙化灶、坏死/出血、分隔、骨髓浸润/骨膜反应、皮质破坏、软组织影、周围组织结构侵犯)的显示均优于X线平片及CT横断位。MPR诊断符合率高于前两者(χ2=9.15,P〈O.05)。结论64层螺旋CT多平面重建成像为股骨近端骨肿瘤及肿瘤样病变的诊断提供了有用信息,是股骨近端骨肿瘤及肿瘤样病变的最佳检查技术之一。Objective To study the diagnostic value of 64-slice spiral CT multiplanar reconstruction(MPR) in tumor and tumor like lesions of the proximal femur. Methods X-ray plain film and 64-slice spiral CT were performed in 20 cases of tumor and tumorlike lesions of the proximal femur, and the original data were reorganized by MPR. The capability of the features of the lesions and the diagnostic coincident rate with pathological results were compared among X-ray plain films, axial CT images and MPR. Results MPR could display the extent of the lesion and the characters( including the hardened edge, tumoral bone or calcification, necrosis or hemorrhage, septum, bone marrow infiltration or periosteal reaction, cortical destruction, soft tissue and the surrounding tissue structure invasion) ,which were better display than that on X-ray plain films and axial CT images. The diagnostic coincident rate of MPR was higher than that on X-ray plain films and axial CT images( χ2 = 9. 15, P〈0. 05). Conclusion 64-slice spiral CT MPR can provide useful information, and is one of the optimal choices for the disgnose of tumor and tumor-like lesions of the proximal femur.
