感染性眼内炎早期诊治分析

摘　　要：目的分析感染性眼内炎的早期诊断及治疗方法。方法回顾性分析46例（46只眼）感染性眼内炎患者，对17例轻度眼内炎患者行玻璃体腔内注药，其中2例病情继续发展行玻璃体手术；对29例重度眼内炎患者行玻璃体切除联合玻璃体腔内注药术，术前均常规抽取玻璃体行病原学检查，分析比较两组患者治疗前后视力、眼压变化及眼球保留情况。结果46例感染性眼内炎均得到控制，眼球保存率100％。16例玻璃体培养结果阳性，阳性率34．8％。行玻璃体腔内注药组注药前后视力比较差异有统计学意义（z=2，649，P〈0．05），注药前后眼压比较差异无统计学意义（t：2．040，P〉0．05）；行玻璃体切除联合玻璃体腔内注药组手术前后视力比较差异有统计学意义（z=3．395，P〈0．05），手术前后眼压比较差异无统计学意义（t=0．107，P〉0．05）。结论感染性眼内炎早期诊断及正确治疗能挽救眼球，使视功能不同程度的恢复。Objective To analyze the methods of early diagnosis and treatment for infectious en- dophthalmitis. Methods The retrospective study included 46 patients （46 eyes）, 17 patients with mild infectious endophthalmitis received intravitreal injection. Among them, 2 patients whose disease continued to develop received vitrectomy, 29 patients with severe infectious endophthalmitis received vitrectomy combined with intravitreal injection. Vitreous body was taken in all patients before sur- gery for etiological examination. The visual acuity, intraocular pressure and eye retention situation be- fore and after treatment were comparatively analyzed. Results Infectious endophthalmitis was con- trolled in all patients after treatment. The rate of eye retention was 100%. 16/46 （34.8%） vitreous samples were positive for culture. In the group of intravitreal injection, the visual acuity improved significantly after injection （Z =2.649, P 〈0.05） and there was no significant difference of intraocular pressure before and after injection （t =2.040, P 〉0.05）. In the group of vitrectomy combined with in- travitreal injection, the visual acuity improved significantly after surgery （Z =3.395, P 〈0.05） and there was no significant difference of intraocular pressure before and after surgery （t =0.107, P 〉 0.05）. Conclusions Early diagnosis and treatment of infectious endophthalmitis can save eye and re- cover visual acuity inordinately.
