摘  要:目的探讨多形性胶质母细胞瘤(GBM)预后相关的临床因素。方法回顾性分析160例随访资料完整的GBM,采用Kaplan-Meier法计算生存率及进行单因素分析,Log-rank法进行生存率显著性检验,Cox比例风险回归模型作多因素分析。结果本组病人术后1年、2年、5年生存率分别是65.3%、34.1%、9.1%。单因素分析显示年龄、肿瘤切除程度、治疗方式是影响GBM生存率的因素。多因素分析显示年龄、肿瘤部位、肿瘤切除程度及治疗方式是与GBM预后相关的独立因素。结论年龄、肿瘤部位、肿瘤切除程度和治疗方式是影响GBM预后的主要因素,积极的手术治疗和规范化、个体化的综合治疗可有效提高GBM病人的生存率。Objective To investigate the clinical factors influencing prognosis of glioblastoma multiforme(GBM).Methods The clinical data of 160 GBM patients were analyzed retrospectively.Kaplan-Meier method was used to calculate the survival rates and analyze the univariate factors for the survival rates.The univariate analysis was used to determine the prognostic factors related with survival rate by Log-rank test.Multivariate factors for the survival rates were analyzed by the Cox proportional hazards regression model. Results The overall 1-,2-,and 5-year survival rates were 65.3%,34.1% and 9.1% respectively.Univariate analysis revealed that age, tumor resection extent and treatment modality were the factors influencing survival rate of GBM.Cox multivariate analysis showed that age,tumor location,tumor resection extent and treatment modality were prognosis-related independent factors.Conclusions Age, tumor location,tumor resection extent and treatment modality are main factors influencing prognosis of GBM.The combination of aggressive surgical treatment and standardized and individualized treatment can effectively improve the survival rate of GBM patients.
