胃癌根治术后复发类型及影响因素分析

摘　　要：目的胃癌根治术后复发的类型及其影响因素,为预防胃癌根治术后的复发提供科学依据。方法回顾性分析75例胃癌根治术后复发者的复发类型、时间及其原发性胃癌的病理类型,同时采用logistic回归分析影响其术后复发的因素。结果 75例胃癌根治术后复发患者中,腹腔内复发者68例,占90.7%,排在前3位的分别是腹腔淋巴结转移者38例,占55.9%;残胃及吻合口复发者13例,占19.1%;肝脏转移者7例,占10.3%。浆膜有无浸润、淋巴结阳性枚数、TNM分期均与胃癌根治术后复发密切相关,均为胃癌根治术后复发的高危因素,而术后7～8周的化疗则是胃癌根治术后2年内复发的保护性因素。结论相对准确的TNM分期、胃癌根治术后规范的术后化疗、加强术后2年内的随访,是提高胃癌患者术后生存率的关键因素。Objective To explore the recrudescent patterns and factors of gastric cancer patients who underwent radical resection,and to provide a scientific basis to prevent the recurrence of gastric cancer after radical surgery.Methods Recrudescent type,time and pathological types of 75 cases of recrudescent gastric cancer patients were retrospectively analysed,the influencing factors of the recurrence were analysed by logistic method.Results Among 75 patients who receipted gastric cancer radical surgery,there were 68 cases of abdominal cavity,accounted for 90.7%,38 cases of abdominal lymph node metastasis,accounted for 55.9%;13 cases of residual stomach and anastomotic recurrence;accounted for 19.1% 7 cases of hepatic metastases,accounted for 10.3%.Depth of invasion,lymph node positive number,TNM stage were closely associated with recurrence of gastric cancer.They were risk factors of recurrence of gastric cancer after radical surgery,and postoperative chemotherapy in 7,8 weeks were the protective factors of recurrence in patients with gastric cancer who underwent radical surgery in 2 years.Conclusion Relatively accurate TNM staging,and standard postoperative chemotherapy of gastric cancer patients who underwent radical surgery,as well as strengthen the follow-up in 2 years are the key factors to raise postoperative survival rate of gastric cancer patients.
