摘  要:目的探讨雌激素受体α(ERα)、细胞外信号调节蛋白激酶(ERK1)和血管内皮生长因子(VEGF)在人甲状腺乳头状癌各组织学类型中的表达及其相关性。方法采用免疫组织化学链霉亲和素-过氧化物酶(SP)法对59例甲状腺乳头状癌(PTC),56例甲状腺腺瘤和30例正常甲状腺组织中ERα、ERK1和VEGF的表达进行检测。结果 ERα在PTC、甲状腺腺瘤和正常甲状腺中的阳性表达率分别为83%、59%和30%,差异有统计学意义(P<0.05);ERK1在PTC、甲状腺腺瘤和正常甲状腺中的阳性表达率分别为69%、48%和23%,差异有统计学意义(P<0.05);VEGF在PTC和甲状腺腺瘤中的阳性表达率分别为64%和39%,而在正常甲状腺无表达,3组之间差异有统计学意义(P<0.05)。淋巴结转移率较高的高细胞型和弥漫硬化型PTC中ERα、ERK1和VEGF的阳性表达显著高于淋巴结转移率较低的乳头状微癌(P<0.05)。在PTC中ERα与ERK1和VEGF的表达呈正相关(rs=0.486,P=0.000和rs=0.419,P=0.001)。结论 ERα对甲状腺滤泡上皮的增生和分化以及甲状腺肿瘤的血管生成均有促进作用,其过度表达与甲状腺癌的发生发展有关系。Objective To investigate the correlation between the expression of estrogen receptor alpha (ERα), extracellular signal-regulated kinase-1 (ERK1) and vascular epithelial growth factor (VEGF) in different histopathologieal variants in patients of papillar thyroid carcinoma(PTC). Methods To compare and study the expression of ERα, ERK1 and VEGF, which were examined by SP immunohistochemistry, in 59 cases of PTC , 56 cases of thyroid adenoma and 30 cases of normal thyroid tissues. Results The positive expression rate of ERα in cases with PTC, thyroid adenoma and normal thyroid tissue were 83%, 59% and 30%, respectively. There was significant difference between PTC and adenoma or normal thyroid tissue (P〈0.05). The positive expression rate of ERK1 in cases with PTC, thyroid adenoma and normal thyroid tissue were 69 %, 48% 和 23%, respectively. There was significant difference between PTC and adenoma or normal thyroid tissue (P〈0.05). The positive expression rate of VEGF in cases with PTC, thyroid adenoma and normal thyroid tissue were 64%, 39% and negative, respectively. There was significant difference between PTC and adenoma or normal thyroid tissue (P〈 0.01). The expression of ERα, ERK1 and VEGF in PTC were related with the histological variants as well as lymph node metastasis(P%0.05). The ERα expression was positively correlated with the expression of ERK1 and VEGF in PTC (rs = 0. 486, P= 0. 000 and rs = 0. 419, P= 0. 001). Conclusion The ERα may stimulate proliferation and promote differentiation of the thyroid epithelial ceils. The increase of ERα expression may play a role in promoting the angiogenesis and development of PTC.
