不同程度神经功能、肝功能损害的肝豆状核变性患者心理状况分析

摘　　要：目的探讨不同程度神经功能、肝功能损害的肝豆状核变性（HLD）患者的心理状况。方法采用Young神经功能评分标准、Nazer肝功能评分标准以及症状自评量表（Scl-90）对76例住院HLD患者进行测查;比较无明显神经功能损害组（Young评分0~14分）和有明显神经功能损害组（Young评分15~28分）的Scl-90结果;比较无明显肝功能损害组（Nazer评分0~6分）和有明显肝功能损害组（Nazer评分7~12分）的Scl-90结果。结果有明显神经功能损害组不良心理问题发生率为58.5%,远高于无明显神经功能损害组（34.3%,P〈0.05）;有明显神经功能损害组HLD患者的人际关系敏感、抑郁、焦虑、敌对性、恐怖、偏执、精神病性因子均分及Scl-90总分高于无明显神经功能损害组（P〈0.05或P〈0.01）。有明显肝功能损害组HLD患者不良心理问题发生率为70%,远高于无明显肝功能损害组（39.3%,P〈0.05）;有明显肝功能损害组HLD患者人际关系敏感、抑郁、焦虑、恐怖因子均分及Scl-90总分高于无明显肝功能损害组HLD患者（P〈0.01或P〈0.05）。结论神经功能、肝功能损害的程度可能是影响HLD患者心理状况的重要因素,有待今后进行多因素分析研究进一步确定。Objective To study the psychological status of Hepatolenticular degeneration （HLD） patients with different neurologic and liver lesions. Methods We used Young scale, Nazer scale and Symptom Checklist 90 （Scl-90） to evaluate 76 HLD patients. The results of Scl-90 of HLD patients with different degree neurologic and liver lesions were compared respectively. Results The prevalence rates of psychological problems in apparent neurologic lesions group was 58.5%, more than that of unapparent neurologic lesions group （34.3%,P〈0.01）. The prevalence rates of psychological problems in apparent Liver lesions group was 70%, more than that of unapparent liver lesions group （39.3%,P〈0.01）.The total score and the score of interpersonal sensitivity, depression, anxiety, hostility, phobia anxiety, bigotry, psychotism of apparent neurologic lesions group were higher than those of unapparent neurologic lesions group （P〈0.05 or P〈0.01）. The total score and the score of interpersonal sensitivity, depression, anxiety, photic anxiety of apparent Liver lesions group were higher than those of no-apparent liver lesions group （P〈0.05 or P〈0.01）. Conclusion The degree of Neurologic or liver lesions may be important factor that affected the psychological status of HLD patients.
