儿童弱视综合治疗802例临床疗效分析

摘　　要：目的评价儿童弱视综合治疗的远期疗效。方法回顾性分析802例（1370眼）经综合治疗并随访3a以上的弱视儿童病历，按弱视类型、开始治疗的年龄、弱视程度、注视性质各分为3组进行研究。按弱视类型分：屈光不正组579眼、屈光参差组76眼、斜视组715眼。按开始治疗的年龄分：3—6岁组740眼、7—8岁组486眼、9～12岁组144眼。按弱视程度分：轻度组610眼、中度组600眼、重度组160眼。按注视性质分：中心注视组850眼、旁中心注视组504眼、周边注视组16眼。结果802例（1370眼）弱视中基本治愈901眼（65．77％），进步388眼（28．32％），无效81眼（5．91％），总有效率94．09％。弱视治疗效果与弱视类型、开始治疗年龄、程度、注视性质密切相关。各类型弱视中，以屈光不正性弱视疗效最好，各类型之间差异均有统计学意义（均为P〈0．01）。年龄越小，治疗效果越好，各年龄组之间差异均有统计学意义（均为P〈0．01）。弱视程度越轻，治疗效果越好，各组之间差异有统计学意义（均为P〈0．01）。中心注视组疗效最好，各组之间差异有统计学意义（均为P〈0．01）。结论儿童弱视采用综合治疗可以取得较满意的效果。] Objective To evaluate long-term efficacies of comprehensive therapy on children amblyopia. Methods Case histories of 802 patients（ 1370 eyes） who had received comprehensive therapy and been followed up for more than 3 years were retro- spectively analyzed. According to amblyopic type, all subjects were divided in to three groups, ametropic amblyopia group （ 579 eyes ）, anisometropic amblyopia group （ 75 eyes）, and strabismic amblyopia group （715 eyes）. According to at which year subjects had received their first treatment, they were divided into 3 - 5 years old group （ 740 eyes） ,7 - 8 years old group（485 eyes） ,and 9 - 12 years old group（ 144 eyes）. According to amblyopic degree, they were divided into mild group （ 510 eyes）, medium group （ 500 eyes）, and serious group （ 150 eyes）. According to fixation nature：they were divided into central fixation group （ 850 eyes）, paracentral fixation group （504 eyes）, and peripheral fLxation group （15 eyes）. Results Of all subject eyes, 901 eyes were basically cured （55.77%） ,388 eyes were improved（28.32% ） ,81 eyes were unimproved（5.91% ） ;The total effective rate was 94.09%. The efficacy of treatment was closely related to ambly- opic type, treatment-starting age, amblyopic degree and fixation nature. Efficacy of the treatment on ametropic amblyopia was most marked among all three types. Differences between efficacies of the treatment on each two types of amblyopia were significant（ All P〈0.01 ）. The younger the treatment-starting age had been,the better efficacy of the treatment was. Differences between efficacies of the treatment on each two treatment- starting age groups were significant（ All P 〈 0.01 ）. There was a positive correlation be- tween the mildness of amblyopia and the efficacy of the treatment.
