摘  要:目的:探讨成纤维细胞生长因子受体1(fibroblast growth factor receptor 1,FGFR1)及Survivin蛋白在乳腺浸润性导管癌(invasive ductal carcinoma,IDC)中的表达、意义及两者的相关性。方法:采用免疫组织化学SP法检测81例乳腺IDC组织石蜡标本中FGFR1及Survivin的表达情况。结果:FGFR1及Survivin蛋白阳性表达率分别为29.63%(24/81)和77.78%(63/81)。它们的表达均与患者年龄无关(χ2=0.919,P=0.338;χ2=1.442,P=0.233);但与细胞增殖指数(Ki-67)呈明显正相关(r=0.446,P<0.001;r=0.269,P=0.015);FGFR1的表达与雌激素受体(estrogen receptor,ER)呈正相关性(r=0.285,P=0.010),与孕激素受体(progesterone receptor,PR)、表皮生长因子受体2(human epidermal growth factor receptor 2,HER-2)呈负相关性(r=-0.371,P=0.001;r=-0.372,P=0.001),而与肿瘤大小(χ2=2.222,P=0.329)、腋淋巴结转移(χ2=0.424,P=0.515)、TNM分期(χ2=0.608,P=0.738)和病理组织学分级(χ2=4.514,P=0.105)差异无统计学意义;而Survivin表达与ER的表达(r=-0.145,P=0.195)和PR的表达(r=0.172,P=0.126)无关,与HER-2呈正相关性(r=0.275,P=0.013),并与肿瘤大小(χ2=21.500,P<0.001)、腋淋巴结转移(χ2=4.772,P=0.029)、TNM分期(χ2=10.792,P=0.005)及病理组织学分级(χ2=11.638,P=0.003)密切相关;FGFR1与Survivin表达呈显著正相关,r=0.347,P=0.006。结论:FGFR1和Survivin可能是乳腺IDC患者的不良预后因素;FGFR1及Survivin可能共同促进乳腺癌的细胞增殖、侵袭及转移,联合检测有助于更准确地判断乳腺IDC的预后。OBJECTIVE: To investigate the expressions,significance and correlation between the protein of Survivin and fibroblast growth factor receptor 1 (FGFR1) in invasive ductal carcinoma (IDC) of breast. METHODS: The expressions of Survivin and FGFR1 in 81 cases of IDC were detected with immunohistochemical methods. RESULTS: The percentage of positive expressions of FGFR1 and Survivin in IDC tissues were 29.63 % (24/81) and 77.78 % (63/81) respectively. The expression of FGFR1 and Survivin were no correlation with age(x2 = 0. 919, P= 0. 338; X2 = 1. 442, P= 0. 233 ), but had significant correlation with the proliferation index/Ki-67 (r= 0. 446, P〈0. 001 ; r= 0.269, P = 0. 015). The expression of FGFR1 was positively correlated with ER (r= 0. 285, P= 0. 010), and negatively correlated with PR and HER-2, (r= - 0.371, P = 0. 001, r = - 0.372, P = 0.001 ). It had no correlation with tumor size, axillary lymph node metastasis, TNM stage and histological grade (X2 = 2.222, P = 0. 329 ; X2 = 0.424, P = 0. 515 ; X2 = 0. 608, P = 0.738 ;X2 = 4.514, P = 0.105). The expression of Survivin had no correlation with ER, PR ( r = - 0. 145, P = 0. 195 ; r = 0. 172, P = 0. 126), but had positively relation with HER-2. tumor size, axillary lymph node metastasis, TNM stage and histological grade(r=0. 275,P=0. 013;X2=21.50,P〈0.001;X2=4. 772,P=0.029;;(2=10. 792,P=0.005;X2=11. 638,P= 0. 003). The expression o{ Survivin had a positive correlation with the expression of FGFR1 (r= 0. 347, P = 0. 006). CON- CLUSIONS: The expressions of FGFR1 and Survivin may be a poor prognostic factor in patients with IDC;and it may accelerate cell proliferation,invasion and metastasis of the IDC. Combined detection of FGFR1 and Survivin can help to determine the prognosis of IDC more accurately. [
