摘　　要：目的：回顾性分析经脐单孔法与多孔法腹腔镜肝切除患者的临床资料，探讨两种方法的疗效、安全性、可行性以及患者满意度。方法：收集2009年3月-2012年8月中国医科大学附属盛京医院胆道血管外科腹腔镜肝脏手术27例，其中单孔组13例，多孔组14例。对丽组围手术期资料进行比较分析。结果：全部患者均顺利完成腹腔镜肝切除术。两组患者在平均手术时间CP=0．358）和术中出血量（P=0．595）方面的比较差异无统计学意义。单孔组患者术后肠道功能恢复早于多孔组（12±0．4d vs 1．5±0．5d，P=0．029）。所有患者均于术后8d内出院。术后随访3个月-2年，两组患者并发症的发生率无统计学差异，术后患者满意度评分单孔组明显高于多孔组。结论：经脐单孔腹腔镜肝切除术可以达到与多孔腹腔镜手术相同的疗效，且美容效果好，具有可行性，其应用价值需进一步临床验证。Objective： To study the clinical data of patients who underwent transumbilical sin- gle-incision and multi-incision laparoscopic liver resection, explore the safety feasibility and pa- tients＇ satisfaction of transumbilical single-incision laparoscopic liver resection. Methods： From Mar 2009 to Aug 2012, patients underwent laparoscopic liver resection in Shengjing Hospital of Chi- na Medical University. There were 13 patients in the single-incision group and 14 patients in theconventional group. Collect and compare the clinical data of the two groups. Results：All the opera- tions were successfully completed with a mean operation time of（P=0.358） and blood loss of（P=0.595）. Patients in the single-incision group resumed normal bowel activity earlier than those in the con- ventional group （1.2 ± 0.4 day vs 1.5 ± 0.5 day, P=0.029）. All the patients discharged from hospital within 8 days after operation（6.8 ± 0.8 day vs 7.0 ± 0.9 day, P=0.462）. The postoperative compli- cations between the two groups had no significant difference after a period of 3 to 48 months. Conclusion： Transumbilical single-incision laparoscopic liver resection is safe and feasible while its clinical application needs further study.
