D3淋巴结清扫术对进展期胃癌患者预后的影响研究

摘　　要：目的探讨D3淋巴结清扫术对进展期胃癌患者的疗效。方法对96例进展期胃癌患者分别行D2、D3淋巴结清扫术。结果观察组和对照组术后1～3年生存率比较无差别,但4、5年生存率对照组分别为56.3%和39.6%,观察组分别为64.6%和52.1%,两者比较有明显差异(P<0.05)。对照组和观察组术中出血量、术后住院时间、并发症发生率和淋巴结转移率比较无明显差异(P>0.05),但对照组手术时间[(185±20)min]明显短于观察组[(224±25)min](P<0.05)。在生活质量评估方面,对照组和观察组各项特殊症状评分指标比较差异无显著性意义(P>0.05)。结论进展期胃癌行D3淋巴结清扫术是安全的,能够显著改善患者的预后且不影响生活质量。Objective To study the influence of D3 lymph node dissection on prognosis of gastric carcinoma. Methods 96 patients with advanced gastric carcinoma were subjected to D2 or D3 lymph nod dissection. Results No significant difference was found between the 2 groups in the 1 -to 3-year survival rates（P 〉 0.05 ）. But the 4-and 5-year survival rates of Group D2 were significantly lower than th^se of Group D3 ,which was 56.3% vs 64.6% ,39.6% vs 52. l% ,respeetively（P 〈0.05 ）. The bleed- ing amount, hospital duration, complication rate and lymph node metastasis rate of the 2 groups had no significant （ P 〉 0.05 ）. The operation time of D2 was shorter than D3, ~ （ 185 ＋- 20 ） rain I vs [ （ 224 ＋_ 25 ） rain ], P 〈 0.05. There was no significant differ- ence in qualily of lit~ between the 2 groups. Conclusion D3 lymph node dissection is safe to patients with advanced gastric car- cinoma, and it can improve the prognosis effectively without affecting quality of life.
