社区综合干预对慢性精神分裂症患者复发次数影响的对照研究

摘　　要：目的探讨社区综合干预对慢性精神分裂症患者临床症状和复发次数的影响。方法分层随机选取100例患者分为研究组和对照组,每组50例,研究组提供社区综合干预,对照组给予一般社区服务,干预前和干预1 a后采用阳性与阴性症状量表(PANSS)、社会功能缺陷筛选量表(SDSS)对2组患者进行评定,分析2组复发率。结果 2组患者干预1 a后PANSS评分比较差异有统计学意义(t=-3.51,P=0.02),研究组PANSS总粗分的改善优于对照组;SDSS评分比较差异无统计学意义(t=0.17,P=0.37)。研究组患者复发率低于对照组,差异有统计学意义(χ2=7.36,P=0.00)。结论社区综合干预有利于控制和改善社区慢性精神分裂症患者精神症状,减少复发次数。Objective To study the effect of community-based comprehensive intervention on relapse frequency and clinical symptoms in patients with chronic schizophrenia. Methods One hundred patients were stratified randomly divided in- to intervention group （n = 50 ） and control group （n = 50 ）. Patients in intervention group were regularly provided with communi- ty-based comprehensive intervention for one year, while the patients in control group were only received common services. All patients were assessed by instrument including positive and negative symptom scale （PANSS） and social disability screening schedule （SDSS） before and one year after intervention. Relapse frequency was analyzed between the two groups after one year. Results One year after intervention, there was significant difference in the score of PANSS （ t = - 3.51, P = 0.02 ） between the two groups. The total score improvement of PANSS in intervention group was significantly better than that in control group. Compared with control group, there was not statistical difference of SDSS score in intervention group （ t = 0.17, P = 0. 37 ）. The relapse frequency of intervention group was significantly lower than that of control group （X2 = 7.36,P = 0.00）. Conclusion The community-based comprehensive intervention is benefit to relieve the symptoms, reduce the recurrence.
