老年乳腺癌患者术后上肢水肿的相关因素分析

摘　　要：目的探讨影响老年乳腺癌患者术后上肢水肿的独立危险因素。方法回顾性分析2009年4月～2012年5月在南昌大学第二附属医院接受手术治疗的294例老年乳腺癌患者的临床资料,根据术后患者上肢有无水肿分为两组,观察组术后上肢出现水肿,对照组术后上肢未出现水肿。相关因素分析先采用χ2检验进行单因素分析,然后采用非条件Logistic回归检验进行多因素分析。结果①125例患者术后出现上肢水肿,术后上肢水肿发生率为42.5%;②单因素分析显示两组患者年龄、体质指数(BMI)、肿瘤象限、病理分级、淋巴结转移、手术方式、术后放疗等指标差异有统计学意义(P<0.05);③多因素分析显示BMI、淋巴结转移、术后放疗是影响老年乳腺癌患者术后上肢水肿的独立危险因素(P<0.05)。结论根据相关因素分析结果,应采取相应的干预措施以降低老年乳腺癌患者术后上肢水肿的发生率。Objective To investigate the independent risk factors for postoperative upper limb edema in elderly patient with breast cancer： Methods The clinical data of 294 cases of elderly breast cancer who received surgical treatment in the Secorid Affiliated Hospital of Nanchang University from April 2009 to May 2012 were analyzed retrospectively.Ac- cording to with or without postoperative upper limb edema,these patients were divided into two groups,the observation group had edema,while the control group didn＇t have.The relevant factors were analyzed by ehi-square test and non- conditional logistic regression test. Results （1）125 cases had postoperative upper limb edema,and the incidence rate of this postoperative complication was 42.5%. （2）The chi-square test showed that there were significant differences in the age,body mass index （BMI）,tumor quadrant,pathological grade,lymph node metastasis,surgical approach and postopera- tive radiotherapy between the observation group and the control group （P〈0.05）.（3）The non-conditional logistic regres- sion test showed that the BMI,lymph node metastasis and postoperative radiotherapy were independent risk factors for postoperative upper limb edema in elderly patient with breast cancer （P〈0.05）. Conclusion According to the analysis results of relevant factors,it should take effective measures against postoperative upper limb edema in elderly patient with breast cancer.
