单吻合器双荷包缝合法与双吻合器吻合法在直肠癌前切除术中疗效比较

摘　　要：目的探讨中低位直肠癌患者行直肠吻合的方式及其疗效，从而研究经济、实用、安今的直肠癌低位吻合方式。方法2008年1月～2012年12月行手术治疗的中低位直肠癌患者97例，其中行单吻合器双荷包缝合法46例，双吻合器吻合法51例，采用病例对照研究回顾性分析两组的临床效果。结果单吻合器双荷包缝合组在住院费用、吻合口瘘发生率低于双吻合器吻合组（P〈0．05），但其在吻合口出血发生率、局部复发率上高于双吻合器组（P〈0．05），两组在住院天数、手术时间、术中出血量、吻合口狭窄、术后排尿功能障碍发生率、术后排便功能障碍发生率、术后感染方面的比较，无统计学差异（P〉0．05）。结论在中低位直肠癌根治术中，单吻合器双荷包缝合法与双吻合器吻合法吻合效率相当，但其降低了住院费用，更经济、实用，在经济不发达地区和基层医院应当提倡为首选的吻合方式。Objective To compare the curative effect of colorectal anastomosis made by single stapling double pouch suturing and double stapling for the anterior resection of rectal cancers.We study economic, practical,safe way of colorectal anastomosis.Method The case-control study was applied to analyze the clinical outcomes of the 97 cases with rectal cancer in the time of January 2008 to December 2012,in which 46 cases were undergone by single stapling double pouch suturing,51 cases by double staplers.Results The single stapling double pouch suturing group was lower than the double stapling group of the hospitalization costs and of the rate of anastomotic leakage（P 〈 0.05）,but was higher than the double stapling group of the rate of anastomotic bleeding and locoregional recurrence（P 〈 0.05）.There was no significant difference between the two groups in the duration of hospitalization,in the operative time as well as intraoperative blood loss.Comparing the rate of anastomotic stricture,urethral dysfunction,defecation dysfunction,postoperative infection in the two groups,it also had no statistical difference.Conclusion In the anterior resection of rectal cancers,single anastomating and double pouching stapler and double staplers contribute considerably, but the former reduces the cost of hospitalization,which is more economic and practical.That＇s to say, the economically underdeveloped areas and hospitals should be promoted with the preferred.
