大鼠出血性脑水肿水通道蛋白4表达与磁共振扩散加权成像变化的相关研究

摘  要:目的探索大鼠出血性脑水肿水通道蛋白4与MR扩散加权成像(DWI)信号演变之间的关系。方法选择SD大鼠50只,按照随机数字表分成实验组(n=30)和假手术组(n=20),造模成功后分别于出血后6h、1、3、5、7d进行MRI扫描,检测脑水肿区域的DWI、ADC的变化规律,应用免疫组织化学方法观察脑出血后AQP4的表达。结果 AQP4的表达在出血后6h即上升,3d达高峰,随后逐渐下降,与正常对照组相比差异具有统计学意义(P<0.01);ADC值在出血后6h升高,随时间推移低信号区更加明显且范围扩大,3d达高峰,随后开始下降,但7d仍高于假手术组。AQP4的表达与血肿周围水肿区ADC值呈线型正相关(r=0.763,P<0.01)。结论 AQP4表达变化参与了血肿周围水肿的形成和发展,联合ADC值有助于早期评定出血性脑水肿的类型及演变过程。Objective To investigate the correlation of aquaporin-4 expressions and MR diffusion weighted imaging (DWI) signals in cerebral hemorrhage edema of Rats. Methods Fifty male SD rats were randomized into the sham-operated roup(n = 20)and the hemorrhage group(n = 30). MRI was performed at the 6th and on the 2nd,3rd,Sth and 7th day after model making. Both groups were studied by conventional MRI, and DWI sequence, and ADC in the edema zone was calculated. Immunohisto.chemical staining was performed to observe the expressions of AQP-4. Results AQP-4 expressions rose after 6 hours, reached the peak value and then gradually declined, and there existed statistical differences when compared with the normal control group ( P 〈 0.01). The ADC values rose after 6 hours, the low signal area became larger and more obvious with time, reached the peak value on the 3rd day and then the values began to decline, but still higher on the 7th day when compared with the control group. The AQP4 expressions and the ADC value in edema zone showed linear positive correlation (r =0. 763, P 〈0.01). Conclusion AQP-4 expression changes are involved in the formation of the edema around hematoma. The combined use of ADC values contribute to the early evaluation of the types and evolution process of hemorrhagic brain edema.
