摘  要:目的探讨p53基因在小细胞肺癌组织中的表达及其与bcl-2,bax蛋白表达的相互关系及临床意义。方法选择华东医院病历资料完整的小细胞肺癌的患者50例,正常肺组织16例,用免疫组化SP法检测bcl-2,bax及p53蛋白的表达。结果 p53的表达与bcl-2的表达呈正相关(r=0.213,P<0.05)。p53阳性表达与bax表达无显著相关性(P>0.05)。p53阳性表达在高分化肺癌组织阳性率为17.6%,中分化组织为90.9%,低分化组织为72.7%,三者差异显著(P<0.05)。p53阳性表达在TNM分期中,Ⅰ期、Ⅱ期阳性率27.3%明显低于Ⅲ期、Ⅳ期阳性率89.3%(P<0.05)。结论p53基因的阳性表达对小细胞肺癌的恶性程度和不良预后有明显的临床意义,p53基因与bcl-2通过作用于细胞凋亡的不同阶段,或是bcl-2参与到p53诱导的线粒体介导的细胞凋亡信号通路上,从而在促进肺癌的发生、发展中共同促进,协同作用。Objective To investigate the expression of p53 gene in small cell lung carcinoma(SCLC) and its relationship with bcl-2 and bax expression,as well as their roles in the development and progression of SCLC.Methods 50 cases were obtained from patients with small cell lung carcinoma,whose diagnosis was confirmed by pathology.16 normal lung tissues were used as control.SP immunohistochemical assay was used to detect the expression of bcl-2,bax and p53 proteins.Results The expression of p53 was positively correlated with the expression of bcl-2(r=0.213,P0.05) but not with bax(P0.05).The positive rate of p53 expression in the differentiations of lung cancer tissues were significantly different:17.6%,90.9%,72.7%(P0.05).The upregulation of the positive expression of p53 was related to the TNM stages(P0.05),the positive rate was 27.3% in stage I II and 89.3% in stage III IV.Conclusion The positive expression of p53 gene predicts the more invasion and poor prognosis of SCLC.p53 is correlated with bcl-2 in different apoptotic stages.p53 gene and bcl-2 may play synergetic roles in the pathway of carcinogenesis and progression of SCLC.
