摘　　要：目的固定矫治是当前正畸治疗中应用最广泛的技术,固定矫治器种类繁多,本研究探讨2种不同固定矫治器在尖牙远移时对龈沟液碱性磷酸酶(gingival crevicular flaid-alkaline phosphatase,GCF-ALP)水平变化的影响,为其临床应用提供依据。方法选择30例男性青少年患者,年龄12～16岁,每例患者随机选取同颌同名牙列分别作为实验组和对照组,实验组牙列佩戴自锁直丝弓托槽矫治器;对照组牙列佩戴普通结扎式直丝弓托槽。利用全自动生化分析仪检测矫治前、矫治后1 h、24 h、48 h、72 h、168 h及2周GCF-ALP含量水平的变化。结果佩戴自锁直丝弓托槽矫治器牙列的实验组GCF-ALP含量水平在加力72 h开始升高,升高的幅度、速率均高于佩戴普通结扎式直丝弓托槽矫治器的对照组牙列。结论正畸过程中受机械力的作用GCF-ALP水平发生动态变化。使用自锁直丝弓托槽矫治器,摩擦力较小,相对普通结扎式托槽,GCF-ALP含量水平改变较大。Objective A wide range of fixed appliances can be used for fixed orthodontic treatment which has been applied widely. The aim of this study is to analyze the effect of two different fixed appliances on the changes in GCF-ALP level during distal movement of canines and to explore the characteristics of periodontal tissue remolding during therapy using these fixed appliances. Methods 30 teenager patients （aged 12 - 16 years） were enrolled in this study and every dentition was divided into two groups. The experimental group was treated with self-ligating appliance. The control group was the contralateral dentition treated with straight wire appliance. Gingival crevicular fluid （GCF） samples were collected at 7 points ： before treatment, 1 h, 24 h, 48 h, 72 h, 168 h and two weeks after treatment. The changes of GCF-ALP level were determined by automatic biochemical analyzer. Results The level of GCF-ALP in experimental group was increased after 72 h of treatment. Both magnitude and rate of elevation were significantly higher than control group. Conclusion GCF-ALP level in GCF were controlled by mechanical force during orthodontics therapy. Self-liga- ting appliance induces more change of GCF-ALP level when compared with straight wire appliance. During treatment with self-ligating appliance, the proliferation of osteoblast/osteoclast and periodontal tissue remolding are more active. Self-ligating appliance shows more advantage in osseous tissue remolding.
