320排CT血管成像在肝癌诊断中的临床应用

摘　　要：目的:评价320排CT三维血管成像(CTA)对原发性肝癌(PLC)的诊断价值。方法:103例介入治疗前行全肝320排CT三期扫描及CTA重建的PLC患者,以DSA为参照,分析CTA对肝内瘤灶、动静脉瘘(AVF)诊断的敏感性、特异性和准确性。并对其中31例合并门静脉癌栓(PVTT)患者比较其CTA与DSA的检出率。结果:CTA和DSA对肝内瘤灶的诊断及其血供评价比较无明显差异(P>0.05);CTA诊断AVF的敏感性和特异性均为100%、准确性为97.1%;31例PVTT患者中CTA发现29例优于DSA的22例(P<0.05)。结论:320排CT及其CTA重建可对介入治疗前的PLC作出快速而准确的诊断与评价,具有较好的应用价值。Objective：To evaluate the diagnostic value of clinical application of 320-slice CT angiography（CTA） in primary hepatic carcinoma（PLC）.Methods：All 103 PLC cases underwent 320-slice CT three-phase scan and the CTA reconstruct before interventional treatment,the sensitivity,specificity and accuracy of CTA for detecting hepatic arteriovenous fistula（AVF） and tumor foci in the liver in research group were analysed,and select the merger of PVTT compared with 31 patients with the positive rate of CTA with DSA.Results：CTA and DSA showed that there were no significant difference in diagnostic accuracy and evaluation of blood supply of tumor.The sensitivity,specificity and accuracy of CTA for AVF were 100％,100％ and 97.1％.CTA was superior to DSA for detecting the hepatocellular carcinoma associated with tumor thrombus in portal vein（PVTT） （P ＜ 0.05）.Conclusion：320-slice CT and CTA reconstruct can make rapid and accurate diagnosis and evaluation before interventional therapy of PLC.
