摘　　要：目的探讨CT导引经皮穿刺植入125I粒子治疗肺转移瘤的可行性、疗效及并发症。方法 15例肺转移瘤患者,男9例,女6例,年龄45～80岁,15例患者病灶数为41个,平均直径为2.3cm,125I粒子活度2.59×107Bq(0.7mCi),PD110Gy。术前通过放射性粒子治疗计划系统TPS(treatment planning system)布源,在CT导引下采取粒子间隔1.0cm,后退式平面植入。结果 3月复查15例41个病灶完全缓解(CR)25个,部分缓解(PR)13个,无变化(NC)2个,进展(PD)1个,有效率92.6%。主要并发症为沿穿刺通道少量出血、气胸或迟发性气胸。6个月随访未发生其他严重并发症。结论 CT导引经皮穿刺植入125I粒子治疗肺转移瘤疗效确切、并发症轻,值得进一步探讨。[Abstract] Objective To investigate the feasibility,efficacy and complication of CT guided radioactive seed ^125Iimplanta- tion in treating lung metastasis. Methods 15 patients （9 males, 6 females, the age ranged from 45 to 80 years old） with lung metastasis were enrolled in this study. 15 patients totally had 41 metastatic lesions （mean diameter 2.5 cm）, seed 125 I activity 2.59 X 107 Bq （0.7 mCi）, PD110 Gy. Under CT guidance,^125Iseeds were implanted into lung metastasis according to radioactive particle TPS （treatment planning system）, drawing back plane implantation technique was adopted with^125I seeds at a distance of 1.0 cm. Results 3 months after treatment, among 41 lesions in 15 cases, 25 obtained CR, 13 PR, 2 NC, and 1 PD. The effective rate was 92.6%. The main complications included small amounts of hemorrhage along the puncture channel, pneumothorax and delayed pneumothorax. No other severe complications were seen during 6-month fol- low up. Conclusion CT guided radioactive seed ^125I implantation procedure has good clinical effects in treating lung metas- tasis with few complications.
