联合腹腔干、肝总动脉切除的胰体尾癌扩大根治术一例报道及文献复习

摘　　要：目的探讨联合腹腔干切除的胰体尾癌扩大根治术(改良Appleby手术)的可行性及优越性。方法回顾性分析我院开展联合腹腔干、肝总动脉切除的胰体尾癌扩大根治术1例患者的临床资料,并进行相关文献复习。结果手术时间250 min,术中出血量200 ml,患者术后恢复良好,无出血、胰瘘、肝功能改变等手术并发症。结论联合腹腔干切除的胰体尾癌扩大根治术安全可行,有利于提高胰腺癌的手术切除率,一定程度上提高术后生活质量,延长生存时间。Objective To explore the feasibility and advantage of extend distal pancreatectomy combined with resection of celiac axis and common hepatic artery （modified Appleby surgery）for pancreatic body-tail car- cinoma. Methods The clinical data of a patients suffered pancreatic body-tail cancer performed extend distal panereatectomy combined with resection of the celiac axis and common hepatic artery were retrospectively ana- lyzed. And the associated literatures were reviewed. Results The operative duration was 250 minutes and the intra-operative blood loss was 200 ml. The postoperative recovery of the patient was well without complica- tions such as bleeding, pancreatic fistula and liver function changes. Conclusion The procedure of extend distal pancreatectomy combined with resection of celiac axis and common hepatic artery was safe and feasible, it can increase the surgical resection rate, improve the postoperative quality of life and prolong the survival time of the patient with pancreatic body-tail carcinoma.
