腮腺液、全唾液及血清中CA-50,β_2-MG测定对腮腺肿瘤的鉴别诊断价值

摘  要:用CA - 50IRMA ,β2 -MGRIA放射免疫法对 3 0名健康人 ,64例腮腺肿瘤患者的腮腺液、全唾液及血清中CA - 50和 β2 -MG含量进行分析 .结果 :3 0名健康者上述两项指标各类标本全都为阴性 .恶性肿瘤组CA - 50最灵敏 ,全唾液阳性率为 96 3 % ( 56 7± 2 3 2 )U/mL ,与健康、良性、非肿瘤组比较P值均<0 0 1;患侧腮腺液阳性率为 92 6% ( 4 4 0 3± 2 3 3 )U/mL ,健侧阳性率为 4 0 7% ( 2 3 82± 2 3 4 )U/mL ,健、患两侧分别与健康、良性、非肿瘤组比较 ,P值均 <0 0 1;血清阳性率仅为 7 4 % ( 13 9±2 2 8)U/mL ,与健康组比较P <0 0 5.良性肿瘤组CA - 50含量全唾液、患侧及健侧腮腺液、血清 4项标本阳性率分别为 9 0 % ,0 0 % ,4 5% ,9% .非肿瘤组CA - 50含量全唾液、健侧腮腺液均为阴性 ,患侧阳性率为 2 0 % .β2 -MG指标各组阳性率普遍较低 ,组间差异大多无显著性 ,恶性肿瘤组全唾液、健侧腮腺液及血清阳性率为 11 1%~ 11 4 % ,患侧腮腺液阳性率为 14 8% .良性肿瘤组及非肿瘤组各类标本 β2 -MG阳性率均在 0 %~ 2 0 %之间 .结论 :腮腺局部肿瘤放免诊断指标CA - 50优于 β2 -MG ,全唾液、腮腺液优于血清 ,CA -CA-50 and β 2-MG were analyzed in the parotid secretion, total saliva and serum with the methods of CA-50 IRMA and β 2-MG RIA in 30 healthy adults, 64 patients with parotid diseases ( 27 of them with malignant tumors, 22 of them with benign tumor and 15 of them with non-tumors diseases). All determinations were qualitatively controlled with controlled serum. Results: In normal controls, the values of CA-50(U/mL, x ± s ) are:8 02±4 8 in the total saliva, 7 81±4 2 in the parotid secretion and 5 7±6 1 in the serum, the values of β 2-MG (μg/L) are 1 305 8±176 in the total saliva, 1 321 4±144 3 in the parotid secretion and 1 507 2±103 in the serum, respectively. (positive value :CA-50>20 U/mL; β 2-MG>2 068 μg/L). Both values above mentioned in the 30 healthy subjects were normal. In malignant group, CA-50 was the most sensitive with 96 3% positive rate in the total saliva(56 7±23 2), as compared with the healthy, benign and non-tumors groups the P <0 01. The positive rate in diseased side parotid secretion was 92 6%(44 03±23 3), and 40 7%(23 8±23 4) in normal side, compared with the other groups the P <0.01, 7 4%(13 9±22 8) in the serum( P <0 05 compared with normal control). In benign tumor group, CA-50 in total saliva was 15 23±16 9. The parotid secretion was 8 24±5 33 in diseased side and 9 82±6 3 in normal side and 7 29±7 8 in serum. The positive rates above four samples were 9%, 0, 4 5% and 9% respectively. In non-tumor group, CA-50 in total saliva, parotid normal side were negative and 20% were positive in diseased side. For β 2-MG, each group had the lower values and there was not obvious difference between them. In malignant tumor group, the positive rate in total saliva, normal parotid side and serum were 11 1% and 14.8 % in diseased parotid side. In benign tumor group and non-tumor group, the positive rates of β 2-MG were between 0~14%. Conclusions: CA-50 is the better radio-immunoassay than β 2-MG in the diagnosis of the parotid
