腮腺良性肿瘤区域性切除的临床应用

摘　　要：目的探讨腮腺良性肿瘤的手术治疗原则。方法将136例腮腺良性肿瘤患者按不同术式分成两组,即传统术式组和改良术式(区域切除)组。对两组患者的手术时间长短、手术切口长度以及术后并发症发生率进行比较。结果传统术式组平均手术时间125 min,改良术式组平均手术时间86 min;传统术式组平均伤口长度18 cm,改良术式组平均伤口长度11 cm。术后两组患者并发暂时性面瘫和Frey综合征的发生率差异有统计学意义(P<0.05),改良术式组优于传统术式组。两组患者术后复发率差异无统计学意义(P>0.05)。结论改良后的腮腺良性肿瘤区域切除术符合现代美学要求、缩短了手术时间,降低了术后并发症的发生率,是一种值得临床推广的术式。Objective To explore the surgery principle in benign tumor of parotid.Methods 136 patients who suffer from benign tumor of parotid were divided into two groups according as the operation method,namely the conventional group and reformative group（also as segmental resection group）.The operation time,length of incision and the incidence of complication were compared between the two groups.Results The average operation time in the conventional group was 125 minutes,while the reformative group was 86 minutes.The average length of incision in the conventional group was 18 cm,while the other was 11 cm.Significant differences were found in the incidence of temporary facial paralysis and Frey syndrome after operation（P 0.05） and the reformative group was lower.No significant difference was found in the postoperative recurrence rate between the two groups（P 0.05）.Conclusions The reformative operative method can shorten the operation time,reduce the incidence of complication after surgery and even meet the requirements of aesthetics.It is worthy of clinical promotion.
