TACE联合超声聚焦刀治疗中晚期肝癌的临床研究

摘　　要：目的探讨TACE联合超声聚焦刀治疗中晚期肝癌的方法及疗效。方法将68例中晚期肝癌患者随机分成两组,对照组34例单纯行TACE治疗,联合组34例行TACE联合超声聚焦刀治疗;比较两组6、12、18及24个月生存率、肿瘤缩小情况及临床疗效与有效率以及肝功能变化等。结果对照组6、12、18及24个月生存率分别为:91.12%、76.47%、58.82%和35.29%,平均生存期14个月;联合组分别为:100%、94.12%、79.41%和52.94%,平均生存期22个月;6个月内联合组CT、MRI提示联合组病灶完全坏死者占29.41%,病灶明显缩小者占61.77%;6个月内对照组CT和MRI提示对照组病灶完全坏死者占17.65%,病灶明显缩小者占38.24%;两组间差异均有显著性(P值均<0.05)。两组临床疗效相比较,差异无显著性(P>0.05);但有效率相比较,差异有显著性(P<0.05);两组术后肝功能变化相比较,差异无显著性(P>0.05)。结论 TACE联合超声聚焦刀治疗中晚期肝癌是一种有效的联合治疗方法,近期疗效显著,明显优于单纯TACE治疗。[ Objective ] To study the methods and curative effect of TACE combined with focused uhrasound sword in treatment of mid-late stage liver cancer. [ Methods ] 68 cases of mid-late stages liver cancer patients were divided into two groups, 34 cases of control group were treated by TACE, 34 cases of united group were treated by TACE combined with focused ultrasound sword; to compare the survival rate of 6 months, 12 months, 18 months, 24 months of the two groups, and to compare the shrink rate ,clinical effects, change of liver function, etc respectively. [Results] The survival rate of control group of 6, 12, 18, 24 months was 91.12%, 76.47%, 58.82%, 35.92% respec- tively, the average survival rate was 14 months; and the survival rate of united group was 100%, 94.12%, 79.41%, 52.94% respectively, and the average survival rate was 22 months; during 6 months, CT, MRT of united group indi- cated that patients whose nidus died completely was 29.41%, whose nidus shfnked obviously was 61.77%, during 6 months, CT, MRT of control group indicated that whose nidus died completely was 17.65%, whose nidus shrinked obviously was 38.24%; the differences of the two groups had statistic significances （P 〈0.05）. To compare the clinical effects of two groups, the differences of two groups had no statistic significances （P 〉0.05）; But to compare the effec- tive rate, its differences had statistic significance （P 〈0.05）; the eomparison of postoperative liver function of two groups had no statistie significanccs （P 〉0.05）. [ Conclusions ] The method that TACE combined with focused ultra- sound sword in treatment of mid-late stage liver cancer is an effective united therapeutic method, it＇s effective in treatment recently and was superior to merely TACE treatment.
