摘  要:目的探索大肠癌患者循环肿瘤细胞中ALDH1、EpCAM及Ki67mRNA的表达临床价值。方法收集56例健康志愿者和55例大肠癌患者术前外周静脉血,从全血中提取单个核细胞总RNA并反转录成cDNA,用荧光定量PCRTaqman探针法检测外周血样本中ALDH1、EpCAM及Ki67的表达。结果大肠癌组ALDH1、EpCAM及Ki67mRNA表达显著高于对照组(P<0.05);ALDH1的表达量与肿瘤浸润程度及淋巴结转移相关,EpCAM的表达量与TNM分期及远处转移相关(P<0.05)。结论荧光定量PCR检测大肠癌患者循环肿瘤细胞中ALDH1、EpCAM及Ki67mRNA的表达作为一种简便可行的分子生物学方法,能为大肠癌患者的分期、预后及个体化治疗提供有价值的信息。Objective To search for the circulating cancer cells which are truly useful for early diagnosis of micrometastasis and precise stages by analyzing the expression of the stem cell markers EpCAM,ALDH1 mRNA and proliferation marker Ki67 in circulating colorectal cancer cells with real-time fluorescence quantitative PCR ( FQ-PCR) . Methods 111 blood specimens were collected from 55 preoperative colorectal cancer patients and 56 health volunteers,total RNA was extracted from whole blood and then was transcribed into cDNA; real-time fluorescence quantitative PCR ( FQ-PCR) using Taqman probe technology was performed to quantify EpCAM, ALDH1 and Ki67 mRNA expression in blood specimens. Results The expression of ALDH1,EpCAM and Ki67 mRNA were significantly increased in the blood of colorectal patients in comparison to the healthy group( P 0. 05) ; the expression of ALDH1 correlated to the tumor invasiveness and lymph node metastasis,EpCAM corre- lated to TNM stage and distant metastasis. Conclusions Real-time fluorescence quantitative PCR assessment for ALDH1,EpCAM and Ki67 mRNA in blood species may provide valuable informations for the precisely staging,prognosis,and adjuvant therapy of colorectal cancer patients.
