摘  要:目的探索免疫指标监测下进行个体化的有效肺动脉介入化疗。方法:自1997年1月至1999年1月, 30例Ⅱ,Ⅳ期肺癌患者先予非根治住减负荷手术,继之以肺动脉介入化疗。于每疗程化疗前一天及化疗后三天取外周血检测血清白介素Ⅱ受体(SIL-ⅡR)。随机分两组,监测组若化疗后比化疗前指标上升大于10%,则于下一疗程化疗中更换化疗方案及调整药物剂量。对照组则按经验用药。结果:比较两组治疗前后 SII.-ⅡR均数的变化.两组的复发转移率及中位生存期,显示均具明显差异。结论:动态监测 SII-ⅡR的变化可有效地监测化疗的效果.并能及时地指导治疗.尽最大可能地确保每一化疗的有效性,并监测预后。这一方法对所有实体瘤的化疗均有借鉴作用。Objective In order to made the intervention of pulmonary artery effective and individual by immu- nity monitoring. Methods: From January 97 to January 99, There were 30 cases whom were lung carcinoma of the clinical stage Ⅲ and Ⅳ. First. all cases weredone the non-radical operation of lightening the burden of the tumor. Then. all were given the chemotherapy by intervening of pulmonary artery. Next, one day before and three days af- ter the chemotherapy. all were detected the solube interleukin-2 recepter (SIL-2R) from peripheral blood.Last. all cases were divided into two groups randomly. the observation group was consist of 15 cases. if the immunity index rose more than 10% after the chemotherapy, the scheme would be replaced and the closage of drugs would be adjusted in the next course of chemotherapy. In the control group, 15 cases were treated in empiric therapy. Results:Cont- rasted the mean change of SIL-2R after and before chemotherapy and contrasted the rates of recurrence and metastasis and the survivor-ship of median in two groups, all index showed distinct difference. Conclusions: This method can monitor the effection of chemotherapy through dynamic observing the mean change of SIL-2R and can ensure the ef- fective of chemotherapy to a maximum by guiding treatment in time. it also can monitor the prognosis and use the ex- perience for reference on other solid carcinorna.
