替吉奥治疗老年晚期胃癌临床观察

摘　　要：目的:观察替吉奥治疗老年晚期胃癌的近期疗效及毒副反应。方法:对44例老年晚期胃癌患者进行随机分组,每组22例。替吉奥胶囊组:替吉奥80 mg/m^2d^(-1),分早、晚2次口服,连续服用4周,休息2周,6周为1个周期,2个周期后评价疗效及安全性。FOL-FOX组:奥沙利铂100mg/m^2,静脉滴注d_1;四氢叶酸200mg/m^2,静脉滴注,d_(1-5),5-氟尿嘧啶500mg/m^2持续静脉泵入12小时,d_(1-5),3周为一个周期,2个周期后评价疗效及安全性。结果:替吉奥胶囊组:CR 1例,PR 9例,SD 8例,PD 4例,总有效率45.5%;FOLFOX组:CR 0例,PR 7例,SD 8例,PD 7例,总有效率31.8%;两组有效率比较差异无统计学意义(χ~2=0.863,P=0.353)。两组患者疾病控制率分别为81.82%、68.18%,差异无统计学意义(χ~2=1.091,P=0.296)。替吉奥组消化道反应、外周神经毒性发生率均低于FOLFOX组(P<0.05),替吉奥组与FOLFOX组Ⅲ度~Ⅳ度不良反应发生率分别为4.3%和27.3%,两组差异有统计学意义(χ~2=4.247,P=0.039)。结论:替吉奥治疗老年晚期胃癌疗效肯定,较目前常规化疗方便、安全、患者耐受性好。Objective： To evaluate the effect and toxicity of S-1 capsule for elderly patients with advanced gastric cancer. Methods： Forty-four elderly patients with advanced gastric cancer were randomly divided into 2 groups. The S- 1 groups included 22 patients who were treated by S- 1 80mg/m2/d, twice daily, for four weeks, with intermittent time of 2 weeks, 6 weeks as one cycle. FOLFOX group included 22 patients who were treated by Oxaliplatin lOOmg/m2 at dayl, intravenously, CF 200mg/m2 from day 1 to day 5, intravenously ,5-FU 500mg/m2 （ 12 hr-eoutinous infusion）from day 1 to day 5. Three weeks as one cycle. The effect and toxici- ties in the two groups were evaluated after 2 cycles of chemotherapy. Results： In the S-1 group, one got complete remissions （CR）, 9 partial remissions（PR） ,8 stable diseases（SD） ,4 progressive diseases（ PD）, the total response rate was 45.5%. In the FOLFOX group, no complete C R,7 PR,8 SD,7 PD, the total response rate was 31.8%. The efficacy was not significantly different between these two groups（ X2 -- O. 863 ,P = O. 353 ）. The disease control rate of the S- 1 group and the FOLFOX group was 81.82% and 68. 18% , re- spectively. The difference was not statistically significant（ ~2 = 1. 091 ,P = 0. 296 ）. The rates of gastrointestinal reactions and peripher- al neurotoxieity of S- 1 group were lower than that of the FOLFOX group （ P 〈 0. 05 ）. The incidence of ]]I- IV degree adverse reactions of S- 1 group and FOLFOX group was 4. 3% and 27.3% respectively with statistically significant difference（ X2 =4. 247 ,P =0. 039）. Conclusion： S-1 capsule is effective and tolerable in the treatment for elderly patients with advanced gastric cancer.
