摘 要:目的探讨乙酰肝素酶(Hpa)和增殖细胞核抗原(PCNA)在肝癌(HCC)细胞中的表达及其与临床病理特征和肝癌患者预后的关系。方法采用免疫组织化学方法检测96例肝癌及癌旁组织中Hpa和PCNA的表达,分析两者的表达水平与肝癌临床病理特征的关系,并用Kaplan-Meier生存曲线及Log-rank法检测不同Hpa、PCNA表达组的生存差异,最后以单因素及多因素Cox回归模型分析影响肝癌患者预后的独立危险因素。结果 Hpa和PCNA在肝癌组织中高表达,而在正常肝组织中弱表达或不表达。HPA和PCNA蛋白表达水平与肝癌组织分化程度、门静脉瘤栓、肝内或淋巴转移相关(P<0.05),而与患者的性别、年龄及肿瘤大小等因素无明显相关性(P>0.05)。Kaplan-Meier生存曲线发现Hpa和PCNA在肝癌组织中的表达强度与患者的生存相关,即表达强者预后差(P<0.01);单因素及多因素Cox回归分析发现Hpa和PCNA的高表达、肝癌组织分化程度、门静脉瘤栓、肝内或淋巴转移等因素是肝癌患者预后的独立危险因子。结论 Hpa和PCNA在肝癌组织中表达升高,Hpa和PCNA高表达与肝癌预后呈负相关,是肝癌预后的独立危险因素。联合检测Hpa和PCNA蛋白的表达有助于判断肝癌生物学行为和评估预后。Objective To investigate the expressions of heparanase (Hpa) and proliferating cell nuclear antigen (PCNA) in hepatocellular carcinoma (HCC) and their correlations with clinicopathological characteristics as well as the invasion, metastasis and prognosis of HCC. Methods The expressions of HPA and PCNA were detected by immunohistoehemistry in 96 eases of HCCs and carcinoma adjacent tissues. Chi-square test, Kaplan-Meier method and multivariate analysis were used to evaluate the relationship between the expressions of Hpa, PCNA and clinicopathological characteristics of HCC. Results HPA and PCNA were over-expressed in HCC tissues,as compared with those in normal liver tissues ( P 〈 0.05 ). The expressions of Hpa and PCNA in HCC were closely correlated to tumor' s differentiation, thrombosis, intrahepatic or lymphatic metastasis ( P 〈 0.05 ). Kaplan-Meier survival curves showed that cumulative survival rates of patients with positive expression of HPA and PCNA were significantly lower than those of patients with negative expression ( P 〈 0.01 ). Conclusion Hpa and PCNA are over-expressed in HCC tissues,however,which is negatively correlated with patient's prognosis and is the independent risk factor of HCC prognosis. The combined detection of the expression levels of Hpa and PCNA is helpful to evaluating the biological behaviors and prognosis of HCC.
