泛影葡胺联合大承气汤治疗急性胰腺炎合并肠麻痹疗效观察

摘　　要：目的评价泛影葡胺联合大承气汤治疗急性胰腺炎合并肠麻痹的效果。方法将86例急性胰腺炎合并肠麻痹患者随机分为治疗组44例和对照组42例,对照组给予常规胃肠减压、抗感染、抑制胰酶分泌等治疗,治疗组在此常规治疗基础上给予泛影葡胺及大承气汤胃管注入,观察2组患者肠鸣音恢复时间、肛门排气时间、排便时间及腹胀缓解时间,第1,3,7天测定白细胞计数、血淀粉酶、腹内压及APACHEⅡ评分,比较2组变化情况。结果治疗组白细胞计数、血淀粉酶、腹内压及APACHEⅡ评分下降幅度明显大于对照组(P均<0.05);肠鸣音恢复时间,肛门排气、排便时间及腹胀缓解时间较对照组明显缩短(P均<0.05)。结论泛影葡胺联合大承气汤治疗急性胰腺炎合并肠麻痹有显著疗效。Objective It is to observe the curative effect of meglumine diatrizoate combined with Dachengqi decoction on acute panereatitis with enteroparalysis. Methods 86 cases of acute pancreatitis with enteroparalysis patients were randomly divided into two groups,the treatment group（ n = 44） and the control group （n =42）. The control group was treated with basic therapy, such as gastrointestinal decompression, anti-infection and inhibition of pancreatic ferment secretion. The treatment group was treated with meglumine diatrizoate combined with Dachengqi decoction stomach tube injection together with the basic therapy. The recovery of borborygmus, anal exhaust time, defecation time, abdominal distension remission time and the count of white blood cell, hemodiastase,intra-abdominal pressure, APACHE Ⅱ grade on day 1,3,7 were observed in both groups, and the changes were compared between the two groups. Results The decline range of count of white blood cell, hemodiastase, intraabdominal pressure, APACHE Ⅱ grade of the treatment group were sharply more obviously than those of the control group（ P 〈 0.05）. The recovery of borborygmus, anal exhaust time, defecation time, abdominal distension remission time of the treat- ment group were significantly shorter than those of the control group （ P 〈 0.05 ）. Conclusion The treatment with meglumine diatrizoate combined with Dachengqi decoction for acute pancreatitis with enteroparalysis has a significant curative effect.
