摘 要:目的:探讨参麦注射液对脓毒症大鼠心肌细胞凋亡及Bcl-2、Bax蛋白表达的影响。方法:体质量280~350 g的健康清洁级雄性SD大鼠45只,随机分为假手术组(Sham组)、脓毒症模型组(CLP组)及参麦注射液治疗组(SMI组),每组15只。采用盲肠结扎穿刺法(CLP)造模。CLP组大鼠腹腔内术前1 h注射0.9%氯化钠溶液10 mL/kg。Sham组不结扎、不穿刺盲肠,其他操作同CLP组。SMI组术前1 h腹腔内注射参麦注射液10 mL/kg。术后立即皮下注射50 mL/kg林格氏液抗休克。术后24 h取大鼠心肌组织,TUNEL法检测心肌细胞凋亡,免疫组化检测Bcl-2、Bax蛋白的表达。结果:模型复制中CLP组死亡6只,SMI组死亡4只,最终Sham组15只,CLP组9只,SMI组11只进入统计学分析。大鼠心肌细胞凋亡在CLP组和SMI组高于Sham组(P<0.01),SMI组低于CLP组(P<0.05)。Bcl-2蛋白表达在CLP组和SMI组均低于Sham组(P<0.01),SMI组表达高于CLP组(P<0.01)。Bax蛋白表达在CLP组高于Sham组(P<0.01)和SMI组(P<0.05),SMI组与Sham组差异无统计学意义(P>0.05)。Bcl-2/Bax比值在CLP组和SMI组均低于Sham组(P<0.01),SMI组高于CLP组(P<0.01)。结论:参麦注射液可以通过上调Bcl-2蛋白表达和下调Bax蛋白表达,从而上调Bcl-2/Bax的比值,减少脓毒症大鼠心肌细胞凋亡。Objective: To investigate the effects of Shenmai injection on cardiomyocyte apoptosis and Bcl-2/Bax protein expression in septic rats. Methods: A total 45 male Sprague-Dawley rats (280 - 350 g) were randomly divided into sham operation group (Sham group), sepsis group (CLP group) and Shenmai injection treatment group (SMI group) (each n =15). The sepsis modeling was made by cecal ligation and puncture (CLP) modeling. In CLP group the rats were injected with 10 mL/kg saline into their peritoneal cavities 1 hour before CLP. The rats in Sham group were treated the same way except no ligation and puncture of the cecum. In SMI group the rats were treated with 10 mL/kg Shenmai injection fluid into their peritoneal cavities 1 hour before CLP. All rats were given subcutaneous injection of 50 mL/kg Ringer's solution for anti-shock immedi- ately after operation. 24 h after operation, the rat myocardium specimens were collected. The cardiomyocyte apoptosis was detected by using TUNEL method. The Bcl-2 and Bax protein expression was examined by immunohistochemistry. Results: 6 rats were died in CLP group and 4 rats were died in SMI group. Finally, 15 rats in Sham group, 9 rats in CLP group, 11 rats in SMI group were put into the statistical analysis. The myocardial apoptosis index in CLP group and SMI group were higher than that in Sham group (P 〈 0.01), while that in SMI group was lower than that in CLP group (P 〈 0.05). The expression of Bcl-2 protein in SMI and CLP group were significantly lower than that in Sham group (P 〈 0.01). It increased in SMI group compared to CLP group (P 〈 0.01). The Bax protein expression in CLP group was higher than that in Sham group (P 〈 0.01) and in SMI group (P 〈 0.05). Compared with the CLP group, the Bcl-2/Bax ratio in SMI group was upregulated (P 〈 0.01). Conclusion: Shenmai injection could reduce apoptosis in the septic rats myocardium by increase the Bcl-2 protein expression and decrease the Bax protein expres
