摘  要:目的评价改良脑-硬膜-动脉血管联通术(EDAS)治疗炯雾病的效果。方法同顾性分析14例行改良EDAS治疗的炯雾病病人的临床资料,其中缺血性烟雾病9例,出血性烟雾病5例。比较其手术前后的症状及影像学变化。结果14例获得随访4~28个月,平均14.3个月,术前症状均明娃好转或无进展,无新发症状。行DSA复查6例,MRA复查8例,9例缺血性炯雾病改善7例,5例出血性烟雾病改善2例。均无手术相关并发症发生。结论改良EDAS用于治疗炯雾病效果肯定,特别是对缺血性烟雾病效果良好;且该术式手术时间相对较短,术后并发症少,Objective To evaluate the efficacy of modified encephaloduroarteriosynangiosis (EDAS) for Moyamoya diseases. Methods Clinical data of 14 patients with moyamoya diseases treated by modified EDAS were analyzed retrospectively, including ischemic moyamoya disease in 9 patients and hemorrhagic moyamoya disease in 5. The differences of symptoms and images before and after operation were compared. Results Fourteen patients were followed up for mean period of 14,3 months, ranging from 4 to 28 months. The preoperative symptoms were obviously improved or maintained, and no new symptoms occurred. DSA reexamination was performed in 6 patients and MRA reexamination in 8. The images of 7 patients with hemorrhagic moyamoya diseases and 2 patients with ischemic moyamoya diseases exhibited improvement. No complications correlated to operations occurred. Conclusion The modified EDAS is effective with shorter surgical time and less comolications for moyamoya diseases, especially for ischemic moyamoya diseases.
