腮腺多形性腺瘤与Warthin氏瘤的超声图像对比分析

摘　　要：目的探讨腮腺多形性腺瘤与Warthin＇s瘤的超声表现特点。方法回顾性分析经病理证实的43例腮腺多形性腺瘤（年龄15-72岁，平均41．5岁）与33例Warthin＇s瘤（年龄23～69岁，平均51．6岁）患者术前超声影像资料。比较两者在肿块部位、形态、边界、包膜、内部回声均匀度、有无钙化、分隔及血供分布上的不同。结果多形性腺瘤共43个病灶，Warthin＇s瘤共47个病灶，两者在形态、边界以及血供分布上无统计学差异，多数为圆形或类圆形，边界清晰的病灶。但在回声强度、分隔和钙化上有明显差异，多形性腺瘤中低回声比例高于Warthin＇s瘤（81．4％vs36．2％，P〈0．05），分隔出现率低于Warthin＇s瘤（7．0％vs48．9％，P〈0．05），钙化出现率多形性腺瘤高于Warthin’s瘤（11．6％vs0．0％，P〈0．05）。结论腮腺多形性腺瘤和Warthin’s瘤在常规超声表现上有一定的相似性，但在病灶呈无回声且伴分隔时，应首先考虑为Warthin＇s瘤。Objective To discuss the manifests of ultrasound in parotid gland pleomorphic adenoma and WarthinPs tumor. Methods Pre-operational ultrasonography image findings of 43 patients with parotid gland pleomorphic adenoma and 33 patients with Warthinrs tumor, which confirmed by pathology, were retrospectively analyzed. The average age of patients was 41.5 years （range 15-72 years） in parotid gland pleomorphie adenoma and 51.6 years （range 23-69 years） in Warthinr s tumor. The tumor location, shape, boundary, capsule, internal echo uniformity, with or without calcification, segregation and distribution of blood supply were compared for difference. Results A total of 43 loci were found in 43 patients with pa rotid gland pleomorphie adenoma,and 47 loci in 33 patients with Warthin＇s tumor, the majority of loci in both group were round or oval lesions with clear boundary. There was no statistical difference between shape, boundary and the distribution of blood supply. But statistical significant difference was found in echo intensity, separated and calcification. The hypoechoic proportion of pleomorphic adenoma is higher than WarthinPs tumor （81.4% vs 36. 2%, P〈0. 05）, separated emergence rate of less than Warthin＇s tumor （7.0% vs 48.9% , P〈0. 05）, the rate of calcification appeared in pleomorphic adenoma is higher than Warthin＇s tumor （11.6%vs 0.0 %, P〈0. 05）. Conclusions Parotid Pleomorphic adenoma and Warthin% tumor demonstrate similar manifestations in conventional ultrasonogaphy, but in lesions with anecho and separate, parotid Warthinrs tumor should be considered firstly.
