摘　　要：目的探讨乳腺癌患者手术前后外周血淋巴细胞表型CD8、CD28变化及临床意义.方法应用CD8、CD28单克隆抗体,采用流式细胞术,对40例乳腺癌和25例对照组手术前后CD8,CD8+CD28+和CD8+CD28-进行测定.结果乳腺癌CD8的表达(32.8±4.5)%高于对照组(30.4±3.3)%,P<0.05,而CD8+CD28+细胞表达(10.4±1.7)%则低于对照组(14.3±2.6)%,CD8+CD28-细胞表达(25.1±2.8)%高于对照组(19.6±2.3)%,P<0.01.切除肿瘤后,外周血CD8、CD8+CD28-细胞、CD8+CD28+测定值(31.9±4.3)%(15.4±2.7)%(20.4±2.8)%与正常对照组相比无明显变化,P>0.05.手术前后CD8、CD8+CD28-细胞、CD8+CD28+细胞测定值均有差异P<0.05.结论乳腺癌手术切除后,细胞毒性T淋巴细胞免疫防御和免疫监视增强,免疫抑制减弱,减少肿瘤免疫逃逸,对乳腺癌免疫治疗和预后判断具有一定的指导意义.Objective To investigate the expression of CD8 and CD28 lymphocytes in peripheral blood of patients with breast cancer before and after operation and explore its clinical significance. Method Flow cytometry was applied to test the expression of CD8, CD8＋CD28＋ and D8＋CD28- in the peripheral blood of both 40 patients with breast cancer and 25 healthy controls. Results The expression levels of CD8 in patients with breast cancer were significantly higher than those in healthy controls [ （30.4 ±3.3） % VS. 30.4± 3.3） %, P〈0.05] . The expression levels of CD8＋CD28＋ in patients with breast cancer were significantly lower than those in healthy controls [ （10.4 ±1.7） % VS. （14.3 ±2.6） %, P〈0.05] . The expression levels of CD8＋CD28- in patients with breast cancer were significantly higher than those in healthy controls [ （25.1± 2.8） % VS. （19.6 ± 2.3） %, P〈0.05] . After operation, the expression levels of CDS, CD8＋CD28＋ and CD8＋CD28- [ （31.9 ± 4.3） %, （15.4 ±2.7） % and （20.4 ±2.8） %, respectively] in peripheral blood of patients with breast cancer had no significant change compared with the healthy controls. The expression levels of CD8, CD8＋CD28- and CD8＋CD28- in peripheral blood of patients with breast cancer had significant difference between before and after operation. Conclusions After surgical operation, the immune defence and immune surveillance of cytotoxic T lymphocytes can be enhanced, suppression and tumor escape can be reduced. The assays of cell phenotypes CD8 and CD28 might be evaluating the immunal state and prognosis of patient s with breast cancer.
